Hours: 40hrs
Shift: Day shift; 8:00am - 4:30pm
Location: Southcoast Business Center - Fairhaven, MA; Follows a hybrid schedule

ResponsibilitiesThe New Beginnings Program began in 2016, is grant and philanthropically funded, and has grant funding to support operations through June 30, 2030. Position reports to the RN Team Leader under the Management of Executive Director of Integrated Care Management or designee. Perform diversified duties relating to the New Beginnings Program and supporting programmatic grant funding. In collaboration with a multidisciplinary care team, Family Advocate provides culturally appropriate, trauma informed, person-centered care, based on the recovery oriented principles to promote health and wellness through identification of medical and social needs, and the provision of education, advocacy, support and linkage to resources for pregnant and postpartum parents with exposure to substances including opioids, stimulants, alcohol and other substances during pregnancy to promote optimal outcomes for parent and baby. Ongoing care management is provided for up to 3 years postpartum.QualificationsMust be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
