Job DetailsJob LocationMorgantown Office - Morgantown, WVPosition TypeFull TimeEducation LevelBachelor DegreeTravel PercentageUp to 25%Job ShiftDayJob CategoryHealth CareDescriptionNATURE OF WORK: The Family & Children's Case Manager's duties include assessment of consumer's needs and strengths; planning and setting treatment goals and objectives with the client; evaluation of progress toward these goals; advocating on behalf of the client; linking/referring consumers to needed community resources; and crisis assistance/intervention. The F&C Case Manager convenes the treatment team and acts as primary contact person for the consumer within the agency. The F&C serves as an information source and interfaces with external services ensuring quality of availability, continuity, flexibility, and integration of necessary resources and services.ESSENTIAL DUTIES:
- Provide education, linkage, and referral to community resources appropriate to the level of client need.
- Assist with procurement of all services that are appropriate and necessary for each child/family within and beyond the scope of Valley HealthCare System's Family and Children's Services.
- Coordinate Transfer/Discharge meetings and follow-up with other service providers on the provision of treatment to allow for continuity of services.
- Access the necessary resources detailed in a consumer's treatment plan, make referrals to qualified service providers and resources.
- Monitor to ensure that the child/family's health and safety needs are addressed.
- Provide planning and coordination before, during, and after crises.
- Further develop and monitor the progress of the child/family's treatment plan.
- Completes and submits documentation as required by Valley, Medicaid, and any other licensing agencies. Employee ensures that documentation is completed accurately and timely and will reflect services the client(s) has received as appropriate.
- Attends all mandated trainings related to profession and enhances knowledge and/or skills which includes Relias Learning Trainings.
- Adhere to Valley's Standards of Conduct and Compliance and follows ethical guidelines of their profession.
- Staff participates in group or individual supervision with his/her supervisor.
- Meets productivity standards of Valley HealthCare System's Family and Children's Services Program.
- Returns telephone calls and requests for information in a timely manner.
- Serve on committees as assigned.
- OTHER DUTIES AS ASSIGNED.
QualificationsMINIMUM QUALIFICATIONS:
- Ability to perform Essential Duties as outlined below.
- BA/BS in Human Service Field.
- 6 months to 1-year experience with working with families and children in human services or related-training.
- Experience working in behavioral health services.
- Knowledge of community resources for behavioral health problems, including mental illness, substance use, and intellectual/developmental disabilities.
- Experience working with diverse populations.
- Knowledge of agency, state, and federal guidelines.
- Ability to comply with Consumers' Rights.
- Ability to comply with all of Valley's Policies and Procedures.
- Ability to comply with Valley's and Division departmental safety procedures.
- Ability to read, write and speak the English language.
- Ability to perform basic computer and E-Mail functions.
PREFERRED QUALIFICATIONS:
- Experience with services for children and families
