RISE Program Family and Fathers Advocate will focus on key elements of family preservation and reunification, including giving fathers and committed family members a voice in the family engagement process. In this capacity, the RISE Program Family and Fathers Advocate will begin with conducting family needs assessment and coordinate services selected by those willing to participate in the program. The overall goal of the R.I.S.E. program is to provide [through interagency collaboration] a comprehensive approach to programs, services and activities in a way that results in stability, well-being, reunification, and preservation for 125 Pregnant and Post-Partum women (PPW) and their minor children, fathers of the children, partners of the women, and the extended family members of the women and children, as appropriate. ESSENTIAL JOB RESPONSIBILITIES AS REQUIRED BY SAMHSA
- Establish rapport and build relationships with family and fathers in the RISE program
- Apply motivational techniques to engage family and fathers in the RISE program
- Conduct family needs assessment with fathers of the babies, current intimate partners if different from the father, and extended family members.
- Establishes rapport and working relationship with assigned families and fathers
- Coordinate treatment and wraparound services for family and fathers
- Foster opportunities for family engagement activities
- Empowers and encourages families to make use of all identified resources.
- Works with collaborative partners to facilitate access and utilization of services
- Conduct monthly home visits 6-months post treatment
- Familiarize yourself with (and adhere to) the RISE program operating manual and protocols
- Participate in a community of practice modeled after the systems of care framework to engage all collaborative partners in coordinating and integrating services
- Actively engage in weekly staff meetings, monthly collaborative partnership updates, and quarterly performance review convenings
- Contribute to achieving the SAMHSA-required measurable objectives (11 requirements)
- Utilize motivational interviewing in working with fathers and extended family members
- Utilize brief intervention techniques and strategies
- Submit monthly progress reports using program reporting template
- This position does not have supervisor responsibilities.
Requirements MINIMUM QUALIFICATIONS
- Bachelor's degree with at least 18 months of related experience preferred or previous experience with a behavioral healthcare related environment.
- Knowledgeable of the recovery community and resources; and experience and knowledge of father's rights initiatives is a plus.
