Please visit nhsc.hrsa.gov for more information.Job Summary:This position functions as a Family Medicine Physician and is responsible for assisting the Chief Medical Officer and Medical Physician Lead. The position is responsible for the provision of primary health care for persons of all ages. The position follows patient centered medical home (PCMH) guidelines and principles, integration, whole person care.Duties and Responsibilities:Applies advanced specialty knowledge, clinical skills, and expertise to assess patients and develop a list of actual and potential needs/diagnosesResponsible for gathering health history of patients through interview of patient and/or family member and physical examinationIdentifies risk factors through discrimination of normal and abnormal findings obtained through health history, physical examination, laboratory and/or radiological test results, and behavioral assessmentApplies advanced knowledge, skills, and expertise in planning and implementation of patient's care by:Assessment and management of wellness with consultation or referral as neededReferral of unstable, chronic illnessCounseling and education relating to specific health care needs and/or health promoting activitiesCollaboration with physicians and other health care professionals in the provision of careMust maintain professional licenses and professional competency through continuing educationMust maintain patient documentation in a prompt and comprehensive mannerEnsure patient calls are returned and patient follow-up for labs, referrals and pharmacy are handled promptlyResponsible and accountable for improvement of quality measures with expected positive health outcomesOther duties as assignedRequired Skills/AbilitiesAbility to relate to people of all socioeconomic levelsA consumer advocate for quality health care and servicesAbility to respond effectively to the most sensitive inquiries or complaintsAbility to handle multiple projects simultaneouslyConsumer advocate for quality health care and servicesPreferred Skills/Abilities:NoneRequired Education and Experience:Current and unrestricted license by the Texas Medical Board as a Medical Doctor.Doctor of Medicine (MD)Certification in area of practice or eligibility for certificationPreferred Education and Experience:ACLS certificationExperience in the health care field (primary or acute care)Experience working with non-profit organizationsSupervisory experienceSupervision Responsibilities:NonePhysical RequirementsFrequently required to sit, talk, and standFrequently required to utilize hand and finger dexterityThe employee must occasionally lift and/or move up to 50 poundsSpecific frequently vision abilities required by this job include:Close vision, Color vision, Peripheral vision, Depth perception, and Ability to adjust focusWork Environment:BLOODBORNE PATHOGENS EXPOSURE LEVEL: 1 (HIGH)Noise Level: ModerateBenefits:Health Insurance with the employee monthly premium paid 100% by MyCHNDental Insurance with the employee monthly premium paid 100% by MyCHN401 (k) with up to 4% company matchingPaid Time Off accrues 128 hours the first year.
