Job Title : Family Nurse Practitioner Location : Littleton, NH Employment Type : Full-time Job Summary : Virtus Healthcare Staffing is seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our partner medical facility in Littleton, NH. The FNP will provide high-quality, patient-centered care, focusing on diagnosing, treating, and managing both acute and chronic health conditions across the lifespan. You will play a key role in health promotion and disease prevention, ensuring that patients receive comprehensive and coordinated healthcare services. Responsibilities : Conduct comprehensive patient assessments and physical examinations. Diagnose and manage acute and chronic health conditions. Develop and implement individualized patient care plans tailored to patient needs. Prescribe medications and other appropriate treatments based on patient diagnosis and treatment plans. Provide preventive care, including immunizations, health screenings, and wellness services. Educate patients and families on health maintenance, disease prevention, and healthy lifestyle choices. Collaborate with physicians, specialists, and other healthcare professionals to deliver coordinated and comprehensive care. Maintain accurate and up-to-date patient records following regulatory and institutional standards. Participate in community outreach and health promotion activities to enhance public health education and awareness. Stay current with medical advancements, clinical guidelines, and evidence-based practices to continuously improve patient outcomes. Qualifications : Master's or Doctoral degree in Nursing from an accredited program. Certification as a Family Nurse Practitioner (FNP) or equivalent. Active APRN license to practice in the state of New Hampshire. Strong clinical knowledge and experience in primary care. Excellent communication and interpersonal skills to foster effective patient relationships. Commitment to delivering high-quality, patient-centered care. Ability to work both independently and collaboratively within a multidisciplinary team. Experience with community health or working with underserved populations is a plus. Skills and Certifications : APRN/FNP Certification (Required) Master's Degree in Nursing (Required) Strong diagnostic and treatment planning skills Proficiency in electronic health records (EHR) systems BLS/ACLS certification (Preferred) Work Environment : This position will be based in our partner medical facility in Littleton, NH, and will involve working with a diverse patient population. The role may require occasional participation in community outreach programs and health promotion activities within the local community.
