- Patient Care: Provide comprehensive primary care services, including diagnosis, treatment, and management of acute and chronic conditions. Focus on preventive care and health education to promote overall well-being in the community.
- Patient Relationships: Build and maintain long-term relationships with patients, ensuring continuity of care and personalized treatment plans. Act as a trusted healthcare provider for individuals and families in the community.
- Collaboration: Work closely with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, to deliver coordinated and integrated care. Participate in team meetings and contribute to the development of patient care strategies.
- Health Education: Educate patients on health conditions, preventive care, and treatment options. Empower patients to take an active role in managing their health through informed decision-making.
- Documentation: Maintain accurate and up-to-date medical records, ensuring compliance with all regulatory standards. Document patient interactions, treatment plans, and progress notes thoroughly and promptly.
- Community Involvement: Engage with the local community through outreach programs, health fairs, and educational seminars. Serve as an advocate for health and wellness in Penn Yan and the surrounding areas.
Qualifications:
- Board Certification: Must have current and active Board Certification as a Family Nurse Practitioner.
- NYS License: Must hold a valid and unrestricted New York State (NYS) License to practice as a Family Nurse Practitioner.
- Experience: Prior experience in a primary care setting is preferred, with a demonstrated ability to manage a diverse patient population across the lifespan.
- Clinical Skills: Proficiency in conducting physical exams, ordering and interpreting diagnostic tests, developing treatment plans, and prescribing medications.
- Communication: Strong interpersonal and communication skills, with the ability to connect effectively with patients, families, and healthcare team members.
