It is not necessary to complete a separate credentialing packet for each position to which you apply.Please follow the link to complete the credentialing application: Please complete either the licensed provider credentialing application (TSCA) or the Non-licensed provider credentialing application as appropriate for the position to which you have applied.I) Job PurposeThe Certified Family Partner is very impactful to adolescent and their families by building protective and promotive factors in families which strengthen families and surrounding communities to ensure each family is heard and their individual needs are being addressed and met.II) Essential FunctionsA) In this role is based on lived experience and training the Certified Family Partner serves the Legally Authorized Representative (LAR) for children and youth served in the Family Services programs of the Child & Family Services Division. During the process the (CFP) will not work directly with youth. B) Adhere to all process, procedures, best practices of MHMR. In addition, to all State and Federal aspects regarding this role. (i.e., contacting assigned families monthly, Recovery Plan (PCRP), Nurturing Parenting and education on TBRI as needed, YES Waiver program transfer warm handoff).C) Communicating and engage with all internal/external programs to provide continuity of care. (i.e., SNAP programs, Good Banks, Help Me Thrive for Community resources, help LAR prepare for and attend IEP, 504, or other school meetings to advocate for the child). In addition, provide peer mentoring, education, and support services to the LAR.D) Performance standards are performed as applicable with MHMR's We CARE values We Connect People in Our Community. E) Performs other job duties or responsibilities as requested or assigned.III) Knowledge of Laws, Regulations, Policies/Procedures, Skills, and AbilitiesA) Knowledge of typical, atypical, and disordered development, and medical diagnoses and conditions impacting children and youth prenatally through age 17.B) Knowledge of CFS program eligibility requirements and services provided.C) MHMRTC Policies and ProceduresD) FERPA and HIPPA complianceE) Certified Family Partner rules and regulationsF) Communicate effectively verbally and in writing.G) Analytical and time-management skills or ability to be flexible and productive at a fast pace and produce quality work.H) Knowledge of protective factors and family -strengthening strategiesI) Ability to multitask, maintain focus, and complete assigned tasks within defined timelines.J) Use of coaching and/or motivational interviewing and trauma-informed approaches while building capacity of children, youth, families, co-workers, and teams.K) Knowledge of Texas laws regarding reporting adult and child abuse/neglect.L) Working knowledge of community resources and case management principles. IV) Internal & External Customer ServiceA) Interact and respond using a compassion mindset, awareness of need for felt safety, and consideration of all variables affecting families and teammates. B) Working effectively with families/adults from various cultural, linguistic, socio-economic, cognitive, social-emotional, and educational statuses.C) Build and maintain effective working relationships with internal and external customers, including families, characterized by mutual support, open communication, trust, and respect. Always considering what is best for families in decisions about treatment and support. D) Discuss uncomfortable and potentially conflict-producing, stressful topics in a professional and effective manner with internal and external customers and maintain courteous interaction.V) TravelA) Adhere to MHMR Mileage and Travel reimbursement policy and any other aspect regarding travel.B) Daily travel including home, school, and community-based visits for families as assigned. Minimum Requirements VII) Minimum QualificationsA) Minimum Education: High School diploma/GEDB) Defined Education: NoneC) Preferences: Spanish-speaking D) Substitutions: NoneE) Years' Experience: NoneF) Defined Experience: Lived experience as a parent or caregiver with a child/youth with emotional or mental health conditions and navigating systems of care.G) License/Certifications: Able to earn on the job certification as a Certified Family PartnerH) Special Courses: NoneI) Supervisory Experience: NoneVIII) Agency RequirementsA) All staff are required to participate in agency Emergency Preparedness and Environmental Safety programs and may be assigned by their department as a key/essential staff level function during critical events or for the purpose of sustaining business continuity.B) This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and/or the Division/Director.C) Assigned work hours may change as the needs of the agency change.D) The Functional Title of this position may change as the needs of the agency change.E) All work will be completed within the scheduled work hours. All non-exempt (hourly) employees are expected to clock in and clock out for each work shift, no work should be done off the clock.F) MHMR reserves the right to change, add to or eliminate positions as it deems appropriate. G) Employment is at will, as well as agency needs may change.H) Agency dress code is to be followed at all times.I) Physical on-site presence, including regular attendance and punctuality, is an essential function of this position.
