Their services include a full scope of primary care offerings, dental care, behavioral health, and more. They are currently hiring due to growth at their newest location in Montrose.POSITION DESCRIPTION: The new Nurse Practitioner or Physician Assistant (APP) will provide comprehensive medical services for patients of all ages. Shifts are scheduled in 10-hour blocks, with 8 hours designated for patient care and 2 hours dedicated for administrative time each day.APPs see an average of 17 patients per day, with the support of dedicated clinical assistants - the clinic schedules 3 Medical Assistants per 2 providers.Specific duties of the role include, but are not limited to:- evaluation and treatment of patients- care plan management- assessment of acute needs- chronic condition management- review laboratory and other test results- initiate specialist referrals- participate in care team, provider, and staff meetings - provide health education to patients and familiesThe office has behavioral health and dental providers on-site, with patient navigators to assist as needed with any needs that arise with the vulnerable patient population that they serve.
