Job Description:

SSM Health is seeking a compassionate and dedicated Nurse Practitioner or Physician Assistant to join our healthcare team in St. Charles County, Missouri. The FNP will provide high-quality primary care services to patients of all ages, emphasizing health promotion, disease prevention, and patient education. The ideal candidate will be committed to delivering patient-centered care and improving the overall health of the community.

Key Responsibilities:

  • Conduct comprehensive health assessments, including patient histories, physical exams, and diagnostic tests.
  • Develop and implement individualized care plans based on patient assessments.
  • Diagnose and manage acute and chronic illnesses.
  • Prescribe medications and other treatments in accordance with state regulations and practice guidelines.
  • Provide preventive care, including routine check-ups, immunizations, and health education.
  • Collaborate with physicians, specialists, and other healthcare professionals to ensure continuity of care.
  • Educate patients and families on managing medical conditions and promoting healthy lifestyles.
  • Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
  • Participate in quality improvement initiatives and professional development activities.

Qualifications:

  • Master s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
  • Current licensure as a Nurse Practitioner or Physician Assistant in the state of Missouri.
  • Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
  • Minimum of 2 years of experience in a primary care setting preferred.
  • Excellent clinical, diagnostic, and therapeutic skills.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively in a team-oriented environment.
  • Proficiency with electronic health records (EHR) systems.

Work Environment:

  • Full-time position with regular office hours.
  • May require occasional evening or weekend shifts based on patient needs.
