Nurse Practitioner (NP)

Titan Placement Group is searching for a Nurse Practitioner (NP) in Louisville, Kentucky.

This position is with a physician-led, mission-driven primary care organization committed to providing comprehensive and compassionate care to seniors. The organization s mission is to honor seniors with VIP care that delivers better health outcomes, supported by services such as no-charge patient transportation to ensure access to care.

Salary and Benefits:

• Salary: $140,000
• Bonus: Up to $10,000 in the first year, with potential increases as a partner
• Paid Time Off
• Comprehensive benefits package, including health, dental, vision, and supplemental plans for providers and their families
• 401(k) with a 5.5% match

Job Duties:

• Schedule: Monday to Friday, 7:30 AM to 5:00 PM (last patient scheduled at 4:15 PM)
• Patient Load: Average of 18 patients per day
• Patient Panel: Small, capped at 450 patients
• Work/Life Balance: No nights or weekends; 100% outpatient care
• Administrative time included in the schedule
• Dedicated care and support team, including a personal MA and scheduler
• Patient Population: Seniors with 3-5 chronic conditions
• Focus on value-based, outpatient primary care

Clinic Highlights:

• In-house consulting specialists in Cardiology, Podiatry, and more
• Holistic health services, including acupuncture, Tai Chi, yoga, and nutrition
• Door-to-door transportation for patients

Requirements:

• Board Certified in Family Medicine or Geriatric Medicine
