Service Coordinators conduct screenings, facilitate eligibility determination, organize and participate in evaluations and develop plans for care. Service Coordinators link families with community resources, teach families how to be effective advocates for their children, and assist families in arranging for services their children may need at the time of discharge. Service Coordinators demonstrate excellent documentation, organization and customer service skills.SKILLS AND QUALIFICATIONS:The Family Service Coordinator must have:1. Knowledge of the local service delivery system, the target group's needs, the need for integrated services, and the resources available or needing to be developed2. A Bachelor's Degree in a human service related field (Human service related fields are those that include coursework in areas such as human development, psychology, social services or similar areas) 3. Three years or more of experience as a Family Service Coordinator (or equivalent case management position) preferred4. One (1) year of supervised experience working with families with special needs5. A valid Wisconsin driver's license, reliable transportation and car insurance are required6.
