Join Our Mission

The Family Specialist is responsible for providing case management services to youth and families consistent with agency mission, vision, and values as well as OCFS regulations.

Job Responsibilities

+ Ensures all case management duties are delivered in accordance with contractual obligations, agency policies and regulatory requirements.

+ Coordinates family contact and visitation which may include home visits with families.

+ Develop Individual Crisis Service Plans and assist staff in dealing with difficult youth and adhering to the ICSPs.

+ I nterfaces with internal and external programs/stakeholders including but not limited to county caseworker/probation for purpose of treatment planning.

+ Responds to crisis situations including de-escalating and engaging youth, processing incidents and/or developing safety plan.

+ Coordinates with local school district to obtain youth schoolwork and supports facility teacher

+ Ensures youth has adequate medication; coordinates with appropriate providers for any medication needs

+ Responsible for facilitating transportation to and from appointments including but not limited to providing said transportation

+ Accurately completes, maintains and tracks all mandatory documentation in accordance with agency policies and regulatory requirements.

+ Accompanies to contract required appointments.

+ Responsible for conducting social-emotional-educational groups

+ Enters appropriate information into database utilized within agency policies and requirements

+ Attends all required internal and external meetings, supervision, and team meetings and accurately prepares required documentation for all meetings. Represents agency in a professional manner.

+ Adheres to agency policies and procedures; promotes and supports agency mission, vision and values including participating in internal surveys and data collection for continuous improvement initiatives.

+ Demonstrated interpersonal skills including ability to listen, act as a role model, and build therapeutic relationships. Demonstrated ability to work effectively one on one with individual youth as well as their family unity.

+ Ability to develop and maintain collaborative working relationships with internal and external stakeholders. Ability to attend, participate and represent agency in treatment related meetings including but not limited to court hearing and family visits.

+ Abi lity to implement change including ability to help youth progress towards discharge goals.

+ Demonstrated written and verbal communication skills required including listening and consultation skills.

+ Demonstrated organizational, time management and prioritization skills required.

+ Demonstrated computer literacy skills including MS Office Suite and other database programs.

+ Handles confidential and sensitive information with extreme discretion, demonstrated knowledge of HIPAA regulations .

+ Ability to work independently as well as in a team environment in a fast-paced culture; must be able to adapt to changing environment.

+ TCI & First Aid/CPR certifications required.

+ Must have a valid driver's license. Travel is required.

+ Ability to work with economically and culturally diverse population.

+ As a Together for Youth employee, you may be required to support other staffing needs in other locations and/or programs as directed.

Job Requirements

+ Bachelor's Degree in related field required.

+ Masters Degree in Counseling, Psychology, Social Work or a related field, license eligible preferred.

+ Mi nimum 1-3 years of experience working with at risk youth and families.

Salary Range & Schedule

+ $23.08/hr - $25.75/hr

+ Monday to Friday (8:00 AM to 4:00 PM)
