What You'll Be DoingYou will provide weekly therapeutic mentoring care management services to foster children; this includes advocating on behalf of the child and their family, providing counseling referrals, coordinating services within the community, and monitoring progress in each case to ensure services are delivered appropriately.Your Main PrioritiesWhile additional responsibilities are likely to arise on the job, you will primarily:•Meets with clients weekly, or as needed to determine quality of care, evaluate additional service needs, and ensure the child's safety and needs are met. •Plans, facilitate, and implement treatment plans through team approach, review reports and plan quarterly meetings. •Develop case plans for clients within proper guidelines, provide case management services to support treatment goals and permanency. •Adhere to all treatment model documentation to assure fidelity to treatment model. •Maintains clients' electronic records, updating accurately and promptly. •Assists with obtaining relevant assessment data (i.e., school records, assessment summaries). •Attends client's medical and educational appointment as needed; attends court reviews and makes recommendations to the court for client care and permanency planning. •Transports clients according to state and agency transportation policies. Who You AreRequirements, Skills & Abilities:•Bachelor's degree in social work or related field required.•Minimum 2 years' experience working in child welfare and/or family services required. •Experience working in the foster care system preferred.•Bilingual in Spanish strongly preferred. •Strong organizational, critical thinking and time-management skills.•Basic computer skills required; knowledge of Microsoft Office 365.•Ability to communicate and effectively interact with a diverse population.•Collaboration and teamwork. •Must have a valid driver's license, proof of auto insurance and car registration. •Must authorize Child & Family to complete a motor vehicle records check. •The ideal candidate will be required to meet all background checks and medical clearances required for the position. Physical RequirementsDirect Service Staff (Travel as part of position):This position requires time sitting, standing, walking, driving, carrying and lifting up to 25lbs. This role also requires operating desktop, laptop and/or cell phone, and communicating with colleagues.
