This is a full-time, permanent position that works a 35-hour week schedule, Mon-Fri w/some evening as needed
Education/Experience Requirement: MSW or related MA degree w/2-years of experience in child welfare/juvenile justice/residential care preferred or a minimum of one full year of professional experience in child welfare, case management. *Valid NYS Driver's License and personal transportation is essential for this role.
Brief Duties Overview: The Family Team Conference (FTC) Facilitator is an intricate support to the multi-systemic service team of the agency, identifying needs and assessing appropriate tasks to achieve stated needs for resident youth and their families. The FTC Facilitator is the lead for groups to formulate tasks and appropriate goals to address and assist in achieving permanency for resident youth. Leads the group in the formulation of tasks and goals for the next six months towards the permanency plan, helping facilitate who, what and when to accomplish next steps. Also liaises with ACS Family Team Conference Team in scheduling FTC meetings that are led by the team.
