Job Description:
Essential Duties & Responsibilities: The Fatherhood Coach is responsible for providing education,support and resources for fathers and father figures of families enrolled in Healthy Start services toreduce infant deaths and improve health outcomes for families. Activities include enrolling fathers intoprogram, providing case management services, facilitating fatherhood curricula one-on-one in thehomes and in the community and collecting evaluation data.Specific Activities:Fatherhood Coaching•Collaborate and coordinate with Healthy Start program to recruit, engage and retain fathers•Facilitate fatherhood curricula including, but not limited to 24/7 Dad, Safe Sleep, and ShakenBaby Syndrome, etc.•Coordinate home visits, workshop schedule, instructional materials and supplies•Ensure fathers complete intake process to enroll into program•Provide holistic and comprehensive services to all fathers including: intake assessment, benefitassessment, goals setting, advocacy and referrals•Regularly engage fathers in the program so that members complete the program requirements,which may include - parenting education, infant mortality education and wraparound servicesand referrals•Practice effective techniques for father engagement; demonstrate and encourage fatherfriendliness within the agency and other community institutions•Maintain caseload to meet enrollment and completion numbers reported at team meetings•Monitor progress of participants on a regular basis and document all encounters•Connect fathers with needed and available community resources•Follow-up with fathers and agencies as appropriate to document use/success of referrals•Ensure that all documents submitted on behalf of a father are valid•Provide all required documentation in a timely matter, which may include father follow-upoutcome evaluation, client contact sheets, and evaluationsEvaluation/Data Entry•Follow data collection protocols for reporting data; input accurate data within prescribedtimeframes.Program Special Events •Plan, coordinate and implement quarterly gatherings for fathers in program. •Plan, coordinate implement and attend special events like the Father 's Day barbecue competition, Barbers for Babies, infant mortality awareness events, etc. Administrative •Attend all staff and team in-services, conferences, workshops and community meetings •Participate in grantee initiatives. Community Outreach •Promote community awareness regarding the value of involved fathers •Network within the community to provide resources for fathers Other: •All other duties as assigned by the Fatherhood Project DirectorEducation and/or Work Experience Requirements:•Bachelor 's Degree in social services or related field or equivalent work experience in the field.•May substitute four or more years of professional experience in community services and/ orcounseling with high risk populations in non-profit sector for degree.Additional Eligibility Qualifications:•Ability to communicate both orally an in writing with individuals for a wide variety of socio-economic backgrounds, including professionals, on-professionals, and the general public,concerning a wide range of subjects and /or situations•Excellent computer proficiency (MS Office Word, Excel and Outlook)•Must be able to work under pressure and meet deadlines, while maintaining a positive attitudeand providing exemplary customer service•Knowledge of the criminal justice system and child support court systems•Ability to work flexible traditional and non-traditional hours•Ability to establish and maintain effective working relationships with Coalition partner agencystaff and the general public•Ability to conduct discussions, interviews, and meetings•Maintain professional boundaries•Experience in making public presentations/public speaking•Ability to maintain confidentiality•Experience working with diverse communities, organizations and agencies preferred•Knowledge of counseling principles and record-keeping techniques•Knowledge of community resources•Experience working with person in crisis•Ability to work independently and to carry out assignments to completion within parameters ofinstructions given prescribed routines, and standard accepted practices.•Must be able to communicate effectively and be a team player at the Coalition.•Must be able to commit to the Coalition vision and be passionate about its mission.•Requires reliable transportation as travel is required, valid driver 's license, good driving recordand automobile insurance.Physical Requirements:•Ability to safely and successfully perform the essential job functions consistent with the ADA,and other federal, state and local standards.•Must be able to travel to alternate locations.•Required to sit, stand, walk, speak and hear while performing the duties of this job, regularly.The position requires extensive computer use so the employee must have sufficient handdexterity to use a computer keyboard and be capable of reading a computer screen•Lifting/carrying up to 25 pounds.Nothing in this job description restricts executive leadership 's right to assign or reassign duties andresponsibilities to or from this job at any time.This position is dependent upon the continuation of funding. As such this position may terminate when the funding is no longer available.
