**Preferred Gold, a fully licensed Private Home CareAgency, is seeking an experienced, Field Case Manager RN** in Brooklyn, NY, who is committed to excellence in client satisfaction, is a strong yet compassionate leader, and is passionate about our mission to ensure patients receive the highest level of care. The Field Case Manger will enjoy being of a dynamic team of dedicated and supportive healthcare professionals.

**POSITION SUMMARY:**

Field Case Manager - RN is responsible for managing the day-to-day clinical operations, planning, organization, and coordination of the healthcare and professional services of the Licensed Private Home Care Service Agency. The Clinical Manager - RN ensures the services they are responsible for are delivered in an efficient and cost-effective manner, maximizing customer satisfaction in conformance with the organizations mission, philosophy and goals. Candidate will ensure all aspects of home care/programs, operate within the standards of all applicable laws, rules, regulations, and upholds Preferred Golds standard of Excellence in patient care at all times.

**QUALIFICATIONS:**

* NYS Licensed and currently registered as a Registered Professional Nurse

* Graduate of an approved school of nursing and holds a Diploma, Baccalaureate or Masters Degree

* Two years of LHCSA nursing experience

* One (1) year of experience with patient care setting, acute care desirable

* Familiarity with LHCSA regulations, preforming chart audits, DOH/Joint commission rules and regulations

* Experience with HHA Exchange

Job Type: Full-time

Pay: $110,000.00 - $120,000.00 per year
