The Intake Nurse will conduct a face-to-face comprehensive assessment utilizing NYSDOH approved assessment tool (UASNY) with a potential member in his/her place of residence. The Intake RN also utilizes CPHL's database of record, completing all necessary documentation to meet CPHL standards. This position requires thorough knowledge of CPHL's program and services, and the ability to communicate clearly the member benefits. The Intake Nurse must possess clinical skills and good judgment, implementing safe plans of care, along with the ability to empathically discuss and describe CPHL services and regulations.PRIMARY RESPONSIBILITIES:Assesses prospects in their place of residence, completing a comprehensive assessment (UASNY) to determine eligibility and initial plan of careEffectively completes the required assessment form (UASNY) and other CPHL necessary documents within the required timeframes and in accordance with CHPL established policies and procedures and State and Federal guidelines and regulations.Communicates plan benefits clearly and easily to prospects and referral sourcesCompletes and submits thorough and high quality documentation in a timely mannerAppropriately alerts Supervisors regarding any issues encountered in the fieldRecognizes quality of care issues and escalates issues appropriately, communicating effectively with coordinators and supervisors as neededComplies with all CPHL's standards for documentationMaintains member confidentiality and compliance with HIPAA requirementsParticipates in regularly scheduled and other staff meetings as requestedParticipates in quality improvement initiativesOther duties as assignedEducation and Experience:EducationGraduate of a 2- 4 year accredited nursing programActive and unrestricted NYS RN licenseType of ExperienceSpecific Technical Skills and KnowledgeStrong written and verbal communication skills.Proficient in the use of Microsoft Office Suite toolsEase with learning new database systemsCertifications/Licensure:Graduate of a 2- 4 year accredited nursing programActive and unrestricted NYS RN licenseMust possess a valid driver's license, motor vehicle insurance and access to a motor vehicle, or access to reliable public transportation within the appropriate service areaAbility to travel within assigned areaProfessional Liability InsuranceSCOPE INFORMATION# Direct Reports: 0Physical Requirements :The physical requirements described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
