Job Description:
We are seeking a dedicated and compassionate Field Registered Nurse (RN) Certified Home Health Aide (CHHA) specializing in adult care to join our client's team. In this role, you will provide essential medical care and support to adult patients within the comfort of their homes. Your responsibilities will include assessing patient needs, developing personalized care plans, administering medications, and assisting with activities of daily living (ADLs) to promote optimal health and well-being.*Key Responsibilities:** Conduct thorough assessments of adult patients' health status, considering physical, emotional, and psychosocial factors.* Collaborate with patients, their families, and interdisciplinary team members to develop individualized care plans tailored to meet the unique needs and preferences of each patient.* Continuously reassess patients' conditions and adjust care plans as necessary to ensure optimal outcomes and quality of life.* Provide skilled nursing care to adult patients, including medication administration, wound care, and management of chronic conditions.* Monitor patients' vital signs, symptoms, and response to treatment, and communicate any changes or concerns to the healthcare team.* Coordinate with physicians and other healthcare providers to ensure seamless continuity of care and timely interventions as needed.* Assist adult patients with ADLs, such as bathing, grooming, dressing, and mobility, while promoting independence and dignity.* Provide guidance and support to patients and their families on adaptive techniques and assistive devices to enhance self-care and mobility.* Foster a supportive and empowering environment that encourages patients to participate in their own care and rehabilitation efforts.* Educate adult patients and their families on disease management, medication adherence, and lifestyle modifications to promote health and wellness.* Provide emotional support and counseling to patients coping with chronic illnesses, disabilities, or end-of-life issues, fostering a compassionate and empathetic approach.* Address any questions or concerns raised by patients or their families, and empower them to make informed decisions about their healthcare options.* Maintain accurate and thorough documentation of all patient assessments, interventions, and outcomes in compliance with regulatory standards and agency policies.* Communicate effectively with interdisciplinary team members, including physicians, therapists, and social workers, to coordinate care and facilitate care transitions.* Participate in case conferences and care meetings to review patient progress, discuss care plans, and identify opportunities for improvement.*Requirements:** Current RN license in the state of practice.* Certification as a Home Health Aide (CHHA) preferred.* Minimum of 6 months years of clinical experience in nursing required.* Strong assessment, clinical judgment, and problem-solving skills.* Excellent communication and interpersonal skills, with the ability to build rapport and establish trusting relationships with adult patients and their families._Please note- The annual base salary range is $90k to $110k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things.
