POSITION SUMMARY: This position serves a vital function in support of the company's clients, seniors and people with disabilities living independently in their homes, but who need assistance with daily tasks.ESSENTIAL DUTIES:Follow specific care plans for clients and report on completed tasksAssist with personal care needs of the client (bathing, dressing, etc.)Provide or assist in routine house cleaning, meal preparation, and laundryTransport client to doctor's office, grocery store and other essential errandsAssist client with the self-administration of medicationsObserve and report any changes in client's conditionPOSITION REQUIREMENTS & COMPETENCIES:Must be 18 years of ageMust be able to pass a criminal background checkMust have reliable transportation; if by car, a valid driver's license and proof of insurance is requiredNurturing and compassionate nature with the desire to care for othersAbility to work with limited supervisionAbility to follow written and verbal instructionsGood communication and interpersonal skillsReliable, energetic, self-motivated and well-organizedMUST have GED/High School degree minimum.FIELD SUPERVISOR ESSENTIAL DUTIES:Conduct Home Care Aide and client monitoring visits in the home to ensure compliance with client care plans on an intermittent or as-needed basis as assigned by Operations Manager.Conduct monthly audit calls for Family Care GiversComplete and submit all required in-home visit forms to Operations Manager/ Agency Director in an accurate, legible, and timely manner.Observe, document, and report immediately any changes in the client condition during the home visit to Change in Condition Hotline. Changes include but are not limited to physical, mental, emotional, financial, environmental, medical, and additional services as needed.Perform in–home follow up on reported complaints or service issues as directed by service coordinator staff.Assist with processing compliance paperwork (alphabetizing and filing) or making compliance calls for four hours each week on a scheduled basis in the office as assigned.Deliver and review Client Welcome Packets and conduct in-home visits with new clients as requested.Assist with in-home training/job shadowing for new home care aides as assigned by Service Coordinator staffAttend all required training per Addus and contractual requirements as well as attend all meetings required by Agency Director.
