We are seeking a Financial Case Manager to join our team and assist clients in navigating the complex world of entitlements and community services. Clients Able to Access & Maintain Benefits
- Assess clients' need for financial and medical benefits and assist them in establishing and maintaining eligibility for benefits.
- Educate clients regarding benefit entitlements.
- Provide financial literacy education tailored to each client's needs.
- Assist clients in establishing a Social Security Representative Payee as indicated.
2. Agency Paid for Services
- Coordinate the establishment of funding in collaboration with Evaluation/Admission Coordinators, Finance Department, and program unit staff to ensure funding is in place at the outset of services.
- Develop sliding scale or repayment agreements with clients as needed.
- Follow up to minimize uncollected debt.
3. Clients Linked to Needed Community Services [OASAS Licensed Program Only]
- Assist clients in searching for safe affordable housing, legal services, selecting healthcare providers, and other essential community services.
4. Documentation Standards Met
- Maintain documentation to the standards defined by regulators, funders, and [Your Organization's Name] policies and procedures.
Physical/Mental Demands:
- Frequent sitting, handling objects and paperwork, computer use, and verbal/written communication.
- Occasional lifting of up to 30 lbs., walking, standing, and reaching.
- Finger dexterity to operate a telephone and computer keyboard.
- Hearing and visual abilities to engage in conversations and read documents.
- Ability to read and interpret policies, procedures, and various other information.
- Ability to maintain emotional control under stressful situations.
Working Conditions:
- Traditional office environment.
- Frequent local travel and occasional transportation of clients may be required.

RequirementsPosition Qualifications:Minimum Education & Experience:
- High School Diploma or equivalent and four years of paid human service experience OR
- Bachelor's degree in a human service-related field and two years of paid human service experience.
Licensure/Certifications:
- NYS motor vehicle license, safe driving record, and availability of a personal vehicle for work.
Other Skills/Knowledge/Experience:
- Demonstrated knowledge of community resources, including social security, public assistance, and Medicaid systems.
- Effective written and oral communication skills.
- Ability to work cooperatively and effectively with culturally diverse clients, staff, and community service providers.
- Basic computer literacy, including internet navigation, email, word processing, and spreadsheet skills.
Salary Description $21-$21.50
