Current licensure to practice professional nursing in the State of Texas. Reliable means of transportation; valid Texas driver's license; automobile insurance. One or more years' experience in clinical nursing. Emphasis on assessment skills and interdisciplinary approach preferred. One year's experience as a case manager for Hope Hospice preferred. Understanding of family dynamics, conflict resolution, and principles of adult learning. Pain and symptom management skills, crisis intervention expertise, and family/patient teaching and counseling skills are required. Suitable personal appearance, behavior, attitude, and integrity. Works well independently. Deals effectively with people. Is a demonstrated team player who is flexible, self-motivated, dependable, and works well with others. Communicates effectively, both verbally and in writing. Computer knowledge required. Critical thinking skills, ability to problem solve, make decisions, set priorities, and keep timely, accurate, and orderly records is required.Duties and Responsibilities:A. Direct Nursing Care, Admissions/Initial Assessment & Care Coordination Respond to patients' and families' hospice needs when the patients assigned RN Care manager is off duty or when assigned to perform duties as a Care Manager. Assesses and documents changes in patient's condition, communicates admission findings, evaluating and appropriately reporting to the patient's Attending physician and/or Medical Director. Schedules patient visits based on acuity, seeking assistance as appropriate. Provide bedside nursing and perform nursing procedures or administer medications and treatments as ordered. Assists the patient, family and caregivers at time of death. Handles arrangements such as: pronouncing death, preparing the body, arranging for funeral home to remove body, notification of appropriate authorities, and medication disposal and equipment removal, in accordance with hospice policy. Provide sensitive listening and communication, discerning when other health professionals should be contacted. Provide and receive nursing reports to designated nurse, reporting any changes in the patient's condition. Documents patient assessments, medications, treatments and responses concisely, accurately, completely at time of care delivery. Electronically completes all communication/documentation accurately, completely, and timely in accordance with policy, rules, and regulations. Participates in nurses' meetings, bereavement meetings, and other activities, as needed. Communicates with IDT members and facility staff members. Appropriately educates patient about hospice services, assessing needs and preferences. Admits patients during normal business hours and weekends, and holidays when assigned. Creates opportunities for new referrals when out in the community. Presents the Hope Hospice programs and services to others in the community, as a member of the agency wide marketing team. B. General Support philosophy and objectives of Hope Hospice. Maintain professional licensure and/or certification current; know and abide by professional practice rules. Stay current with governmental and accreditation requirements affecting area of responsibility. Maintain professional appearance, behavior, attitude, and integrity. Maintain professional competency in area of expertise. Keep abreast of and abide by policies and procedures of the agency. Maintain confidentiality and ethical behavior. Promote a team approach through cooperation, mutual respect, flexibility, and dependability. Participate in appropriate meetings and activities, including Performance Improvement.. Exhibit good stewardship of resources, including self-care. Read materials and attend in-services, seminars or conventions as needed for professional growth. Contribute to an atmosphere of excellence, mutual respect, and caring. Perform other duties as assigned. Working Conditions: Is required to operate a motor vehicle in order to drive to and from assignments in patient homes and nursing facilities. Duties include climbing stairs of varying sizes, types, and degrees that may be slick or in disrepair; negotiating ramps of varying sizes, types, and degrees that may be slick or in disrepair; and dealing with various issues related to the yard and street; and dealing with other circumstances both outside and inside the patients place of residence. Must be able to perform under a wide variety of conditions and circumstances in the home. Must possess sufficient manual dexterity to handle IV procedures, specimens, equipment and medications. May be required to repeatedly lift, carry, push and pull greater than 50 pounds, and carry agency provided electronic equipment for completing the Electronic Medical Record (EMR) in the home. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions resource acquisition. Potential for exposure to blood and body fluids and infectious material. Must be able to communicate in writing and verbally, both in person and by telephone. Also function in normal office environment; moderate amount of standing, walking, stooping, sitting, reaching.
