Food Service Admin Assistant - 188 Days/4 Hr per day (part time)Application StatementBy submitting this application I authorize LaPorte Community School Corporation to check my employment history including, without limitation, evaluations, criminal arrest and conviction record, reference checks, and any investigatory information possessed by any private or public employer or any state, local, or federal agency. It is also understood and agreed that any misrepresentation, by me, in this application will be sufficient cause for cancellation of this application or separation from the employer's service if I have been employed.Before an individual can work with students or have his/her name submitted for approval to the Board of School Trustees, the candidate must complete the background check authorization (at a cost of $15.00--cash or check) and the background check must receive a cleared rating. Additionally, the individual must complete a federal I-9 form (providing appropriate identification documents); tax forms; a direct-deposit form (with an attached voided check); and, view training videos at the Educational Services Center (approximately one hour in length).SCHOOL YEAR: Health, dental, life, LTD insurance; membership in Public Employees' Retirement Fund; 4 paid holidays; 8 sick, 2 family illness, and 4 personal business days awarded annually (prorated for less than a full year of service); severance benefits upon retirement.
