Overview: Our mission at Freedom Communities (FC) is to drive holistic family and community transformation through innovative programs and real estate investments in the Freedom Drive Corridor. Limiting our geographic service area allows us to go deep and work holistically addressing the interconnections of a person's housing, education, mental and physical health, employment and network of support. With a tagline of family-centered community transformation, we invest simultaneously in families and in infrastructure so that as families transform, their community does, too. Research shows that children cannot thrive in homes where their parents struggle economically, and low-income parents cannot succeed without meaningful support for their children. We center our programmatic work around a two-generation approach combining parent and child interventions to build family strength and optimal child development. Our primary demographic are single mothers with children aged 10 and below. We believe they have the most potential to impact the next generation but the least amount of access to resources to do so. We created our Moms Moving Forward (MMF) program in response. MMF is a cash incentive program to drive upward mobility for single mothers living in the Freedom Drive Corridor with at least one child under the age of 10. Our MMF program creates family stability by providing single mothers with the tools, mindset, and support network to transform their whole household. Through our 12-month, cohort-based program, women build sisterhood to help sustain their progress and get paid for their participation, removing barriers and rewarding progress. We employ Transformation Coaches to work with moms and their children to set and meet goals to move their families forward. To date, we have launched 9 cohorts; Cohort 8 will graduate in April.. Family Case Manager Job Description:
- Recruit cohorts of 15+ moms who meet requirements for the MMF Program
- Onboard mothers into the program ensuring that each participant fully understands the program requirements
- Conduct one-on-one weekly meetings with each program participant ensuring effective and timely documentation of each meeting in Apricot (database)
- Partner with participants to identify strengths and barriers
- Work with participants to set and achieve goals to advance their family stability, parenting skills, housing, education, financial stability, employment, and health and wellness
- Attend weekly MMF team meetings / bi-weekly Freedom Communities (FC) team meetings/ attend weekly one-on-one meetings with the direct supervisor
- Maintain documentation of progress against goals set in database
- Connect program participants with local community resources (i.e, housing, education, employment and health & wellness) and maintain communication with the referral organization to ensure followthrough of participant
- Participate in ongoing trainings and membership with EMpath Mobility Mentoring Model and other organizations deemed appropriate by Director of Programs and Impact
- Uphold the values of FC in every interaction with team members, program participants community partners, and members of the community
Youth Development:
- Assess and evaluate the needs of each child and his or her parents as it pertains to that child's success in partnership with schools, health care providers, etc.
- Develop Educational Success Plans (ESP) for each child in the program in partnership with schools and parents.
- Foster collaboration between parents, children, and school to support ESP.
- Maintain accurate records of children's educational progress over the academic school year.
- Support moms by offering resources to support their engagement and involvement in their child's education and development.
Retreats:
- Responsible for working with Community Project Manager, Lead Family Case Manager and Events and Hospitality Coordinator in advance to plan, coordinate, and then execute the various elements relating to the retreats.
- Collect RSVPs for attendance for mothers and children for retreat numbers.
- Communicate with Events and Hospitality Coordinator to secure food and ensure volunteers are in place and aware of their roles and responsibilities.
- Work closely with the Community Project Manager to secure childcare, ensure all medical needs are identified and a plan is in place for emergencies.
- Partner with the Director of Economic Development to secure transportation to and from the retreat ensuring all families have a transportation waiver on file.
- Confirm retreat workshop facilitator(s), and confirm/send reminders to facilitator of date and time of retreat communicating details to Director of Programs and Impact (DPI).
- Serve as retreat host for assigned cohort ensuring the coordination of meals, volunteers, childcare, transportation, etc. and dealing with issues as they arise
