Such care and services include but are not limited to: regulatory compliance, emergencies, memory loss, activities and programming, nursing and health services, dietary, personnel, marketing and customer service, and household management. Caregivers will assure that the highest degree of residential care and services are maintained at all times while preserving the highest ethical standards, the dignity of residents and families. Major ResponsibilitiesMemory Loss
  • Understand and have working knowledge of caring for those with memory loss including but not limited to communication, providing care, managing challenging behaviors, and caring for caregivers.
  • Understand support and carryout PSC philosophy of care and services for those with memory loss.
  • Accept, understand, and treat all residents with dignity regardless of disease diagnosis, progression, and manifestation.
  • Be creative with resident care and services to make individual allowances for disease progression.
  • Other duties as deemed necessary and appropriate or as directed by your supervisor.
  • Dietary1. Assist, supervise and/or prepare meals for residents as required for shift insuring to following proper sanitation.2. Assure meals are prepared in accordance with planned menus, resident needs and special diets prescribed by resident' physician.3. Participate in mealtime to encourage resident socialization and to observe and assist with resident dining skills. 4. Maintain flexibility in breakfast schedule, other individual schedule needs of residents, food likes and dislikes. 5. Encourage resident participation in meal process. 6. Perform necessary logging, monitoring, and measuring as required for special diets and directed by the Clinical Nurse.7. Clean kitchen after each meal and/or snack time following proper sanitation.8. Continue to monitor kitchen for spills, clutter, general cleanliness and potential hazards. 9. Other duties as deemed necessary and appropriate or as directed by your supervisor.Activities & Programming1. Direct all activities and events to meet the needs and desires of the residents in order to ensure the activity-focused care philosophy and to promote mental and social stimulation. 2. Complete the necessary documentation.3. Assist with resident activities to meet resident daily needs. Each day of programming should include gross motor, fine motor, household, 1:1, and memory enhancing activities seven days a week. Outings must be incorporated in the schedule. 4. Encourage resident participation in activities and programming. 5. Other duties as deemed necessary and appropriate or as directed by your supervisor.Nursing and Health ServicesParticipate and complete the necessary training components:Training to be completed within the First 14 Days and Before Direct Care

  • Assure that all resident nursing and health service needs are met to ensure safety, personal hygiene, and positive self-esteem.
  • Assist residents with activities of daily, according to the Assessment & Care Plan, which include:
  • Dressing & Undressing• Bathing• Oral Hygiene• Denture Care• Peritoneal Care• Bathroom Schedules• Toileting• Hair care3. Provide assistance with activities of daily living using good communication skills as related to resident care.
  • Assist residents with vital signs, according to the Individualized Resident Care Plan and as may be ordered by the physician or case manager including:
  • • Pulse• Temperature• Blood Pressure• Respiratory Rate• Weight5. Assist residents with ambulating activities which include:• Lifting & Transferring• Positioning• Range of Motion• Reminders to Use Special Devices• Propelling Wheel Chair 6.   • Observe and document the health and emotional condition of each resident, promptly reporting any changes to the designated person.
  • Maintain residents' records on a daily basis, and in accordance with company policy, procedures, and regulation. This documentation includes:
  • • Medication Distribution• Leisure Activities• Incidents and Observations• Changes in resident's' physical condition and/or behavior
  • Communicate daily via documentation tools shift concerns/events.
  • Provide related duties for resident care and service as assigned by the Cottage Director or Case Manager.
  • Other duties as deemed necessary and appropriate or as directed by your supervisor.
  • Resident Services1. Provide resident transportation as necessary and scheduled.
  • Communicate special problems of resident to Cottage Director or RN Case Manager.
  • Report special and unusual problems to the Cottage Director.
  • Other duties as deemed necessary and appropriate or as directed by your supervisor.
  • Personnel
  • Participate and Complete the following training components and in the following time frames:
  • Training to be completed within the First 14 Days and Before Direct Care
  • Attend and participate in in-service education classes and on the job training programs as directed by the Cottage Director.
  • Participate in scheduled staff meetings on a regular basis; offer advice regarding residents, staff and cottage concerns. Assist in identifying and correcting problem areas and/or the improvement of services.
  • Understand and have working knowledge of the Employee Handbook.
  • Maintain that the contents of your employee file are current.
  • Demonstrate ability to coordinate and prioritize duties on a timely basis to meet needs of residents.
  • Maintain civil and professional relationships with co-workers.
  • Other duties as deemed necessary and appropriate or as directed by your supervisor.
  • WORK ENVIRONMENTEmployee will be exposed to virus, disease and infection from residents and specimens in working environment, employee may be required to work extended periods of time. Incumbent may experience traumatic situations as the resident move through the end of life cycle. PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to focus.This position required intermittent physical activity, including standing, walking, bending, kneeling, stooping and crouching as well as lifting and supporting residents. EXPECTED HOURS OF WORK:This position will be scheduled based on operational need (rotate shifts, mandates, call in).
