Summary of PositionThe Case Manager position will be responsible for providing direct support and assistance in case management to the residents participating in the program(s) and maintenance of residents daily records. The Case Manager will provide support to the Program Director. This position will be responsible for performing all job duties and descriptions according to program policies and procedures. Due to the population being served in this program, we are looking to hire female applicants only.Job Functions
  • Ability to work with youth and/or their families in a changing setting which may include transitional care and aftercare outreach.
  • Coordinate young parent intake: within 2 hours communicate with client and provide them with the programs Welcome Packet, including the programs expectations and requirements. Upon client's acceptance into the program, notify the DCF Young Parent Living Program Clinical Coordinator and DTA Self Sufficiency Specialist worker with the intake plan.
  • Coordinate intake and client's transportation to the program.
  • Complete all intake paperwork within 8 hours of arriving. This includes: Mental Health Status evaluation; immediate needs assessment, Intake Safety Plan; all releases signed, all med documentation, including MAR's, orders, and prescriptions. The client reviews the Welcome Packet and signs the Placement Agreement and all documents in the legal section.
  • Review Intake Safety Plan and Mental Health Status form with a clinician within 8 hours of intake.
  • Maintain all appropriate records according to Caring Together, EEC and Youth Services standards and guidelines.
  • Complete 24 hour Treatment Plan with an LPHA signature. Ensure Youth Readiness Assessment YRAT, is completed within 20 days of intake, and signed by DCF social worker and DCF young parent. Complete initial 30 day Treatment Plan with LPHA signatures.
  • Coordinate treatment team reviews, complete updates to Treatment Plan and obtain signatures. Required every 90 days.
  • Complete CANS (DCF child assessment needs and strengths) assessment within 30 days.
  • Update Safety Plans every 90 days or as needed.
  • Update YRAT every 6 months.
  • Complete Risk Assessment with clinical support as needed.
  • Case Manger is required to learn and use Virtual Gateway to document Treatment Plans, treatment plan reviews, and incident reports for DCF clients only.
  • Required to utilize language in accordance to rehab options expectations.
  • Responsible for the client binders, ensure case file index and subsequent documents. Ensure all documents are signed by young parent and filed properly.
  • Participate in resident team meetings with collaterals
  • Coordinate and record all resident's appointments and daily schedule in Advocate Notes
  • Oversee staff with the documentation and completion of client progress notes, Advocate Notes, and incident reports. Ensure rehab options language is used in progress notes, in addition to collaborative problem solving language for incident reports.
  • Collaborate and communicate updates and important information with DCF, school personnel and any other provider agencies regarding the young parent services and treatment goals. Provide weekly emails to DCF and DTA with summary of progress towards goals.
  • Assure that young parent's quality of life and standards are maintained including: dietary, hygiene, appropriate clothing and building cleanliness.
  • Participate in program support, PAYA (preparing adolescents for young adulthood). This includes parenting skills, job readiness, budgeting, obtaining day care, school completion, housing, permanency work, physical and mental health management.
  • In collaboration with staff ensure groups focusing on safety, and increasing independent living skills are scheduled weekly.
  • Responsible for providing leadership and act as a positive role model for young parents and staff.
  • Required to use personal vehicle, when Centerboard vehicle is not available in order to meet appointment and/or aftercare outreach requirements.
  • Responsible for reading communication log and providing client or program updates in the log as needed.
  • On Call - Participate in the Manager On Call rotation and document calls in Asana.
  • Complete all Assessments/forms in accordance with State/EEC/DCF regulations- i.e. YPLP Assessment, YRAT, CANS, Substance Abuse Evaluation, Mental Status Exams, Treatment Plan, Discharge/Termination forms, Safety Plans, and use of the Virtual Gateway.
  • Collect Program Fees bi-weekly and submit to Fiscal. Chart individual payments and balances in their individual ledgers, found in Google. Place the young parent ledgers in the Program Fee Binder.
  • Conduct In- House trainings for staff- i.e Safe Sleep, IR documentation refresher, BMT refresher, Congruent Care requirements, Progress and Advocate Note documentation, Staff shift responsibilities refresher and others.
  • Perform other duties, responsibilities and related tasks as requested.
  • House Meetings - Conduct a Young Parent House Meeting monthly, and keep the agenda and minutes and attendance in the Groups Binder.
  • Trainings - Keep a log of training hours attended and offered through the Centerboard Track Training list, including outside trainings. These are in person or online. Submit log to HR yearly.
  • Budget - Monitor resident monthly budgets and record budget information on MyEvolv.
  • Discharge/Transition planning - Oversee all communication regarding young parent and their teams, in order to keep next steps in mind. When issues arise, discuss all matters with the DTA/ DCF team, and the PD, and participate in all team meetings. When transitioning a young parent into another YPLP/Adult Shelter develop a plan with the client's team.
  • Maintenance- Add all maintenance repairs and notices on Asana.
  • DCF Monthly Billing- Track the Overnight Log and submit to PD at the end of the month. Complete DCF Assessment form and submit to PD at the end of the month. Complete the DCF Program Fee Log and submit to PD at the endo of the month.
  • Meet with young parents individually once a week. This meeting should include conversations about their Treatment Plan goals, Safety Plan and coping skills. Discuss behavior management and parenting.
  • Complete all referrals for young parents including- Early Intervention, Healthy Families, mental health, education and day care, and Mass Rehab.
  • Housing - Assist young parents in creating a Housing Binder within 30 days. Assist with filling out housing applications and tracking placement on the wait list.
  • Interview New Employee Candidates/Interns - Interview new candidates for hire and interns via FaceTime/zoom/ in person. Complete all HR interview questions. Scan and email HR background paperwork to the candidate/intern and ask them to fill them out and email them back to me along with a copy of their Driver's License. After the interview make a recommendation to accept the individual or not and email everything to the HR department for their review, or drop off the paperwork at the main office in any of the HR mailboxes.
  • Transportation for youth under 18 years old - Staff must transport and stay with a young parent for the following appointments: Hospital/ER visits, collateral meetings, court, school meetings, doctor and dentist appointments.
  • CQI, maintain, track, document, and create information necessary for the individual program. Attend meetings and present information as required. (CQI, Theory of Change).
Qualifications Bachelor's degree in psychology or related field and at least two years of experience working in a residential setting. Case Manager's must have the ability to work flexible hours to meet the needs of clients schedules. Required to pass initial competency evaluation as well as continued assessments of professional development and future trainings. The Case Manager must possess the ability to use collaborative and proactive solutions during behavior management. Qualified candidates must possess strong communication and organization skills and be a willing participant in Centerboard's multicultural awareness, diversity, equity and inclusion trainings, meetings and philosophy.
