Mount Carmel Guild Behavioral Health System, a CARF accredited division of Catholic Charities of the Archdiocese of Newark, is a leading provider of mental health and addiction services in Northern New Jersey.We are currently seeking a Full Time Case Manager for our ICMS - Essex program located in Newark, NJ. (Schedule: M-F 8:00AM-5:30PM, with possible Saturday hours.)The Integrated Case Management Services program (ICMS) provides in vivo case management services to adults with a severe and persistent mental illness with co-occurring substance abuse. Services are designed to engage, support, and link clients to traditional and non-traditional recovery oriented supports and services. The goal is to maximize clients' independence by providing personalized, flexile, and collaborative services in the clients' natural environments.JOB DUTIESTo maintain a caseload of up to 25 consumers and provide linkage and referral services to adults with severe and persistent mental illness in the community.To collaborate with the consumer's family, community, and agencies to ensure the consumer is working towards goals of self-sufficiency.To maintain electronic health records in compliance with state regulations.To provide 50% of direct consumer contact via telehealth and in person visits.Case Managers work from home 2 days per month.REQUIREMENTSBachelor's Degree in a behavioral health discipline1-2 years of related experiencevalid Driver's LicensePreferred experience includes one year of experience in a behavioral health setting.
