Employment Type: Full timeShift: Evening Shift Description: Full-Time On-Call Hospice Visit Nurse The Community Hospice has a wonderful opportunity for a FT on call/ visit RN to work EVENINGS as part of our Hospice Care Team caring for patients and families serving Capitol Region. At times, when necessary-might be asked to assist with patients in Saratoga/Washington/Montgomery/Columbia and Greene Counties. Hours are: 4pm-Midnight and includes an every other weekend rotation Position Highlights:
  • National pre-taxed mileage reimbursement

What you will do:
  • As an RN you will visit patients in the home setting and local nursing homes to respond to symptom management needs
  • Coordinate with an interdisciplinary team to provide added support to patients and families in their homes.
  • This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, LPNs, medical directors and NPs with specialized expertise in end-of-life care, social workers, chaplains, complementary therapists and volunteers).
What you will need:
  • NYS RN Licensure.
  • Associate's Degree in Nursing, BSN preferred.
  • 1-2 years RN experience required-preferably in homecare or acute care.
  • Previous hospice experience preferred but NOT required.
  • The RN must be able to communicate effectively, verbally and written.
  • Valid Driver's License and Reliable vehicle.
  • High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers.
