This job description outlines the objective, primary responsibilities and requirements and qualifications of the Full-time Nurse. Objective: The Full Time Nurse will have the overall responsibility of managing and developing client services for Senior helpers clients with Veteran benefits,Medicaid,Trident Area on Aging and other government paid services This includes meeting goals for retention of clients and managing the growth of hours from current clients. Areas needed to be managed: Charleston,Berkeley, Dorchester, Collenton and Orangeburg counties. Reports To:Branch Manager and Owner Primary Responsibilities (including, but not limited to):
- Utilize professional assessment skills to identify physical,mental,and psychosocial needs of client and be able to develop a care plan to meet these needs in a multi-disciplinary approach and complete all documentation for nursing assessments and evaluations.
- Act on client's behalf to assure patient rights
- Visits all clients on a bi-monthly basis and/or as needed to assess the quality of services as well as the client and family satisfaction and happiness level. Spends approximately 2-3 days per week out of the office re-assessing clients and managing client visits in hospitals,rehabs,facilities,etc. Update the care plan with any changes in client care needs and scheduling.
- Be aware of home safety issues,strive to accomplish all responsibilities in a safe manner,and consistently reinforce good safety attitude and practices displayed by other staff members.
- Ensures that clients and their families are satisfied with Senior helpers services,works to find solutions with the appropriate employees and/or family members
- Work with scheduling and Care Team Manager to ensure clients shifts are staffed with a caregiver with matching skills to ensure clients needs are being meet.
