**Salary Range or Pay Grade**

$195,000 - $228,000 a year

**Description**

**Job Description: Long Term Care Attending**

**A.** **General:**

The Skilled Nursing Facility Attending Physician reports to the Medical Director, UR Medicine Geriatrics Group and is responsible for the complete medical care of all assigned patients.

**B.** **Administrative Responsibilities:**

1. Participates in the administrative and programmatic activities within assigned facilities.

2. Actively fosters, encourages, and participates in Multidisciplinary Team activities in assigned patient areas.

3. Provides oversite to all aspects of patient care and directs floor staff in assigned areas in regard to care related orders and specific patient needs.

**C.** **Patient Care Responsibilities:**

1. Provides long-term medical supervision of patients admitted to the Skilled Nursing Facility according to State, Federal, and hospital policies and regulations.

2. Facilitates Multidisciplinary Team approach to Long-Term Care patients.

3. Is responsible for continuing care of patients - i.e., acts as Attending Physician when patient requires admission to a general hospital.

4. Coordinates care and collaborates with advanced practice providers and facility partners.

**D.** **Teaching Responsibilities:**

1. Actively provides and participates in teaching Geriatrics and Long-Term Care.

2. Provides clinical teaching at bedside to facility staff within assigned facility.

3. Provides direction and supervision to facility staff as necessary.

4. Acts as role model for facility staff by providing and demonstrating expertise in Geriatric Medicine and/or chronic diseases.

**E.** **Research (Full-Time Faculty Only):**

Develops a personal research effort either individually or in collaboration with other faculty members the University consistent with the individual's interest.

**F.** **Qualifications:**

1. Qualified for appointment to full-time faculty at University of Rochester:

OR

Qualified for appointment to part-time faculty with documented strong interest and willingness to participate in academic programs.

2. Board Certified or eligible in Internal Medicine, Family Medicine or Geriatric Medicine.

3. Strong interest in education and teaching.

4. Prefer completion of Geriatric Fellowship and/or demonstrated special interest in Geriatrics and chronic diseases, e.g., demonstrated past experience and/or certification in Geriatric Medicine.

**Qualifications**

M.D. Degree or equivalent

Must obtain a New York State Medical License prior to start date

Eligible for a Faculty Appointment at the University of Rochester School of Medicine and Dentistry

Must meet all credentialing requirements

Eligible for Board Certification in Geriatrics/Family Medicine/Internal Medicine

Post offer Drug Screen and Health Assessment required

Compliance with University and New York State health requirements
