DescriptionWe're looking for an experienced individual to provide guardianship services to clients in the Central and Mid-Eastern part of Ohio.
- Receive and process referrals for guardianship services from Probate Court and service providers.
- Research, prepare and submit required Guardianship documents for filings with Probate Court in a timely manner to remain in compliance with Probate Court rules.
- Prepare for and attend guardianship hearings (with court-appointed attorney when applicable).
- Meet clients face-to-face for initial contact.
- Manage and meet important deadlines for documentation and as required by governing agencies.
- Document all communications and progress notes into case management system.
- Visit clients at a minimum quarterly; more frequent visits may be necessary based upon acuity of case.
- Make consent decisions in accordance with internal policy.
Requirements
- Valid Ohio Driver's License, good driving record, current automobile insurance and reliable transportation required.
- Ability to pass a background check.
- 3 years' experience working in the social services field required, such as aging, mental health, I/DD, or youth.
- Associate degree required, Bachelor's preferred.
- Experience and knowledge of court appointed guardianship requirements, developmental disabilities, Medicaid and waivers, County Boards of DD, community housing, social, human, and mental health services preferred.
- Experience working with persons in crisis using trauma informed and person-centered practices preferred.
- Experience providing or accessing vocational and community resources preferred.
