Education: High School Diploma or GED. Successful completion of orientation training and medication course. Required Experience: 2 years related experience working with people who experience Developmental Disabilities.License: New York State Driver's License and Agency InsurabilityJob Description Summary: The Hab Aide will provide assistance to the teacher in the development, implementation and evaluation of the participant's individualized treatment plan. The Hab Aide is responsible to record data as well as maintain records and charts. In addition, this position is responsible to provide assistance in independent living skills; activities of daily living, cognitive programming, socialization, recreation and communications skills, and also to maintain a safe and hygienic work environment. This position may be responsible to administer medication and to facilitate the utilization of special equipment. Physical Expectations This position requires that employees are able to stand and or sit for prolonged periods of time. The employees must also be able to lift and transport consumers and perform all physical aspects of the job.Essential Duties and Responsibilities include but are not limited to the following: Responsible for the individual's well being. Assist participants with activities of daily living including toileting, grooming, oral hygiene, meal programs, and specialized medical needs as required. May administer medications and records same. May accompany participants on program trips, or to the emergency room, as assigned. Will participate in special program/projects as assigned. Implement participant's individual treatment plans, including recreation, following prescribed methodologies. Monitor participants well being and report any changes in condition and/or health care concerns to the teacher and/or nursing staff. Monitor, prepare, and/or assist in the preparation of participants lunch/snack and report any dietary concern to teacher and/or nursing staff. Implement participant's individual behavior plan, as specified, by following prescribed methodologies.
- Establish and maintain an effective working relationship with team members.
- Participate as an active member of the Day Treatment Team.
- Assist Teachers and Clinical team members in the evaluation of participants.
- Prepare/setup classroom and learning environments for daily activities prior to participant's arrival, as assigned.
- Maintain cleanliness and safety of assigned work area.
- Notify teacher or supervisor of any safety concerns immediately upon becoming aware of them.
- Facilitation in the utilization of special equipment.
- Follow universal precautions.
- Participate in fire and evacuation drills.
- Maintain records and charts for each participant by entering required information (goal/data sheets, behavioral data sheets).
- Complete and respond to all requests for information including Incident Reports.
- All actions should follow those prescribed in the Department Policies and Procedures Manual.
- Attend mandatory in-service and specialized training programs as assigned.
- Demonstrate legal and ethical conduct.
- All actions should adhere to those prescribed in the Agency Employee Handbook or Employees Updates.
- Perform all other duties as assigned.
