This includes the provision of individualized services, as identified in each person's Lifeplan/CFA as specified in the Day Habilitation Plan/Staff Action Plan. These activities and supports are uniquely tailored for and by the person rather than for the benefit of the group. The Day Habilitation Coordinator will ensure that recreational, leisure, advocacy, and social activities are offered, and facilitated in a manner accessible to all. In a manner consistent with the agency mission and applicable laws and regulations, an employee in this position shall ensure progress toward achieving the values of The Day Habilitation Purpose Statement: “Encouraging people to explore, discover and realize their full potential by fostering connections through shared interests while celebrating what makes us unique,” as well as values of the HCBS Medicaid Waiver, i.e., Independence, Integration into the Community, Individuality, and Productivity. In line with CPU's commitment to CARF Accreditation, the employee will demonstrate a commitment to continually enhance the quality of services and programs with a focus on the satisfaction of the persons served.QualificationsBA or BS in an appropriate field of study and experience of at least five years demonstrated ability working with people who have IDD and knowledge of Day Program regulations.Valid NYS driver's license with a clean driving record during employmentProficient with Microsoft officeExcellent organizational skillsExcellent oral and written communications skills.Performs all job responsibilities in full compliance with applicable laws, rules, regulations, policies, and procedures. Adheres to the agency's corporate compliance program, including HIPPA, the code of business conduct, the NYS and federal false claims acts, whistleblower protections, and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
