Job Description:

As a Habilitation Specialist, you will provide Specialized Habilitation Services, Staff and Family Consultation, Stabilization Services, and/or program consultant. Workdays are Monday - Friday.

Note: We are currently looking for someone who can mostly provide Program Consulting services. Specialized Habilitation and Staff and Family consults will be needed as well.

Location: Pierce County and Thurston County of Washington State.

Programs and Descriptions:

Specialized Habilitation: Must promote inclusion in the community and be individualized to the client participant. Support shall be provided with the intent of reaching a habilitative goal identified in the PCSP and in compliance with the Specialized Habilitation Policy. You will create a plan of action which defines our customer's goals, states it in measurable terms, tracks progress, and includes a progress and service report. You will be providing services in facilities and/or at the client's homes. You may work with adults and children.

Supports provided to customers fall under the following five Specialized Habilitation categories:

  • Self-Empowerment - To take control of one's own life by setting goals and making positive choices to promote self-determination. Customers develop positive internal dialogue, build self-esteem, reflect on personal values, and be aware of their own strengths and weaknesses.
  • Safety-Awareness and Self-Advocacy- The ability to recognize hazards and make safe informed choices. Customers learn safety awareness skills. With safety awareness skills, the customer will be able to identify and report possible neglect, exploitation, and/or abuse. We focus on one's ability to self-advocate and learning to say no.
  • Interpersonal Effectiveness and Effective Social Communication- The development of social and emotional learning which leads to positive relationships and self-respect.
  • Coping Strategies Regarding Everyday Life Challenges- To develop problem solving skills and stress reduction strategies to address stressful situations.
  • Managing Daily Tasks and Acquiring Adaptive Skills- To develop necessary skills to thrive in the home and in the community. Skills may include money management and basic understanding, ordering food, and the general understanding of why we do tasks.
  • Staff and Family Consultation: Services are intended to help family members and staff to better provide care for the clients. You will be providing services in facilities and/or at the client's homes. You may work with adults and children.

    Consultation may include:

    • Helping the family or direct service provider properly document medication side effects, changes in health, and behaviors which need to be reported to a prescribing physician.
    • Augmentative communication systems. We will help the direct service provider or family member better assist the client with communication by the use of assistive technology and/or PECS.
    • How to identify resources for the client such as mental health.
    • Helping the family or direct service provider follow a nutritional plan developed by a nutritionist or dietitian.
    • Information on disabilities and educating the family member or direct service provider on the client's disability.
    • Consultation regarding an existing plan of care.
    • Strategies on how to effectively interact with the client. This may include how to consistently use effective communication and de-escalation strategies.
    • Environmental consultation. Helping the family or direct service provider create sensory friendly areas, and environmental modifications to the home.
    • Will develop an initial plan and 90 day report.

    Stabilization: Provides Specialized Habilitation and Staff and Family services to clients who are on the verge of being institutionalized and being expelled from their home due to behaviors. Will develop an initial plan and 90 day report. You will be providing services in facilities and/or at the client's homes. You may work with adults and children.

    Program Consultant: Consulting with an outside agency to help the agency provide excellent care for children.Plays a vital role in program development, staff training, compliance monitoring, and continuous improvement. This position supports the Program Manager with day-to-day operations and ensures all required reporting to the Developmental Disabilities Administration (DDA) is accurate and timely. The Consultant collaborates with staff, families, and external agencies to provide comprehensive care for the children. This position is working only with children. We will be providing consulting services for an outside company.

    Key Responsibilities:

    Program Development and Evaluation:

    • Design and implement programs tailored to the needs of children with developmental disabilities.
    • Develop individualized care plans and monitor their effectiveness.
    • Evaluate program outcomes and recommend improvements based on best practices and feedback.
    • FA/PBSP (Functional Assessment/Positive Behavior Support Plan).
    • Will work with the agency prior to the youth moving in and as needed.

    Staff Training and Development:

    • Create and conduct training programs for staff, interns, and volunteers.
    • Provide ongoing coaching and professional development opportunities.
    • Ensure all staff are knowledgeable about best practices, behavior management, and regulatory compliance.
    • Monitor and document the skill development of staff.

    Compliance and Quality Assurance:

    • Ensure all programs comply with state and federal regulations, including WAC 110-145.
    • Conduct regular audits to verify adherence to policies and procedures.
    • Implement quality assurance measures to enhance service delivery and ensure high standards of care.
    • Review treatment or case plans as appropriate.
    • Consult with or supervise program managers at least one hour for every 40 hours of program management work. Staff consultations must be documented and available for staff to review as needed.
    • Comply with any professional accreditation requirements that apply to the agency.

    Family and Community Engagement:

    • Facilitate effective communication and collaboration with families.
    • Connect families with community resources and support networks.
    • Represent the facility at community events and in professional organizations to build partnerships and raise awareness.

    Behavior Management and Support:

    • Develop and oversee behavior management plans.
    • Provide guidance on crisis intervention and de-escalation techniques.
    • Support staff in managing challenging behaviors and promoting positive outcomes.

    Documentation and Reporting:

    • Maintain accurate records of program activities, assessments, and outcomes.
    • Prepare detailed reports for internal use and external regulatory bodies.
    • Ensure timely and accurate incident reporting and follow-up.
    • Oversee all reporting to the Developmental Disabilities Administration (DDA), ensuring compliance with all regulatory requirements and deadlines.

    Advisory to Program Manager:

    • Provide expert advice and support to the Program Manager in daily operations.
    • Collaborate with the Program Manager to address operational challenges and enhance program effectiveness.
    • Assist in the development and implementation of operational strategies and policies.

    Requirements:

    1. Education:

    • Master's degree in special education, Social Work, Psychology, or a related field from an accredited college or university. A copy of your unofficial transcripts and copy of degree may be requested upon hiring.

    2. Experience:

    • At least 5 years of experience working with children with developmental disabilities.
    • Program consultant must have experience with children who have developmental disabilities. All other programs can have experience with adults and children who have developmental disabilities.
    • Proven experience in program development, staff training, and regulatory compliance.
    • Have experience teaching caregiver's skills to help their client/child. This includes behavioral challenges, coping skills for in and out of the home, self-advocating skills, and other skills or methods that are needed.
    • Working on person-centered programs.
    • Experience in a residential care setting is preferred.

    3. Skills:

    • In-depth understanding of developmental disabilities and effective care strategies.
    • Excellent communication, training, and interpersonal skills.
    • Ability to work collaboratively with a diverse team and external partners.
    • Strong organizational and problem-solving skills preferred.
    • Proficiency in Microsoft Office Suite and electronic health records (EHR) systems preferred. Not all programs will use health records. Only program consultants will use this program.
    • Knowledge of mandatory child abuse , vulnerable adults, and neglect reporting requirements.

    4. Must be able to travel to at least two different locations a day to provide services.

    Certification:

    · Current CPR and First Aid certification. Certification can be obtained when hired.
