We offer a wide range of evidence-based rehabilitative services, including mental health and substance abuse treatment, housing, vocational training and job placement, healthcare, education, and creative arts therapies.The Harm Reduction Coordinator will evaluate and address substance use-related needs to support residents in thriving within supportive housing.Essential Position Functions:
- Provide intensive in-home harm reduction support and/or referral linkages for a small caseload of residents in Bridge housing across the Bronx, Manhattan, and Brooklyn.
- Collaborate with internal departments and external providers to coordinate care.
- Offer harm reduction guidance and training to supportive housing staff.
- Promote physical and mental health, safety, well-being, and recovery for clients.
QualificationsRequired Knowledge, Skills, and Abilities:
- Minimum of a high school diploma or equivalent (GED).
- Experience working within a harm reduction model or with high-risk populations.
- Computer literacy required; ability to use agency Electronic Health Record to document services.
- Bilingual (Spanish) a plus.
