Job Type Full-time Description ** SIGN ON BONUS AVAILABLE - Paid out in 3 equal payments after completion of 3 months, 6 months, and 9 months of employment** This role operates administratively in a Home and Community-Based Services (HCBS) Setting, defined as medical, social, and supportive services for Iowans with functional, cognitive, and other physical or mental health needs (Iowa Health & Human Services). Rather than institutionalizing individuals who have these physical and mental health needs, HCBS settings provide these individuals with the opportunity to live and receive services in a way honoring to them and their abilities. In this role, you'll be responsible to:
- Create and maintain the team culture, which may include:
- Being actively present and involved with your team.
- Conducting monthly team meetings focused on immediate needs and team culture.
- Providing coaching opportunities for staff.
- Agreeing to be placed on-call as required and fill in for the team if there is need.
- In this role, you must be available on short notice to fill gaps in the schedule due to absences, personnel changes, or other unexpected reasons.
- Compensation for on-call duties will be paid in stipends based on rotation, per department guidelines.
- Mileage reimbursement is available when on-call duties require report to work outside of regularly scheduled hours.
- Overseeing resources, which may include:
- Monitoring and scheduling vehicle usage for services.
- Monitoring budgets for individuals served are being maintained and followed as outlined in their financial plan by the Representative Payee Coordinator.
- Supporting individuals in communication with and fulfilling responsibilities associated with their Representative Payee Coordinator.
- Overseeing balances, ledgers, receipts, and special requests forms in collaboration with the Representative Payee Team.
- Monitoring medication check-ins.
- Monitoring appointment scheduling and follow-up communication for individuals served.
- Ensuring services are provided to individuals in a way that honors their needs, desires, and goals, which may include:
- Serving as Imagines' primary point of contact with the Medicaid Community Based Care Manager (CBCM) for individuals served.
- Inputting accurate information regarding the individual served into the approved Electronic Medical Record (EMR).
- Developing individualized plans with the CBCM and Interdisciplinary Team (IDT) that prioritizes the individual's needs and goals.
- Updating essential documents and ensuring authorizations are accurate. In addition, this role may require assessments to be completed for specific services or individuals.
- Conducting quarterly reviews of goal progress for each individual-served under your team's care.
- Communicating with individuals served, their families/guardians, and case managers in a timely manner regarding any changes in the services provided. This includes changes to the kinds of services provided, a limitation of rights, or other concerns regarding the individual served. In this role, you will also be responsible to document these interactions and reasons for changes.
- Collaborating with the IDT to write support goals and a personal outcome plan within 21 calendar days of a new individual being entrusted to our care.
- Ensuring resources, such as ongoing supports, transportation, or other vital components to services provided, are available.
- Collaborate with the Strategic Advancement team to promote local community partnerships and promote services offered by Imagine.
- Demonstrate knowledge, or be willing to learn, and comply with all policies, practices, laws, and rules provided by any governing bodies in the industry.
- Provide complete, consistent, and accurate documentation of incident reports, investigations, and service documentation.
- Be an advocate for individuals receiving services to have as much control over their own lives as possible. All individuals should be treated with respect and should feel empowered to live their life as independently as physically possible.

- Educational Requirement: Minimum Education Requirement of High School Diploma/GED equivalent.
