STATUS: Full Time; 37.5 hours / week

SCHEDULE: Weekdays, 37.5 hours per week Flexible to meet the department needs

Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit, community-based human services agency supporting individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Milestone Centers has earned certification from the Sanctuary Institute for providing trauma-informed care.

Responsibilities:

The primary focus of the services of the Pediatric Complex Care Resource Centers (PCCRC) to be delivered by the Health Care Quality Units (HCQU) will be to support caregivers in the non-medical aspects of supporting a child with complex medical conditions receiving in-home shift care services. Many of these needs may be family-centered and sociodynamic in nature and will lean heavily on advisory, education, and referral functions. Families often require resources outside of what might be found among peers or extended family. The purpose of the services of the PCCRCs are to meet the needs for information and support through focused services to build family and caregiver capacity and efficacy.

The Pediatric Coach is responsible for the provision of services through the PCCRC. The Pediatric Coach will be available at the request of caregiver teams as they care for children who receive shift care nursing services. Caregiver teams include parents, family members, nurses, home health aides and anyone else who may be responsible for caring for a child with medically complex needs.

The Pediatric Coach may provide some number of three (3) distinct functions:

Caregiver Support and Intervention

The Pediatric Coach will be a resource to support principal caregivers in the non-medical aspects of supporting a child with complex needs. The Pediatric Coach may be called upon to provide support and intervention for caregiver teams as they attempt to work together, including problem solving and conflict resolution services as they attempt to collaborate around the needs of the child. They may help to negotiate the bridge between meeting familial social, and medical goals.

Learning Customization and Facilitation

The Pediatric Coach will assist children with medically complex diagnoses by providing their families and health care providers with education and training. The learning support will be targeted to meet each childs specific needs in a way that empowers each childs community to support them in the best way possible. Leveraging learning that is available through the PCCRC and located on the MyODP website, the Pediatric Coach will be responsible for facilitating training including classroom-based, small group, and individualized training.

Family Advisory Workgroup Facilitation

The Pediatric Coach will support caregiver teams by facilitating the meeting of PCCRC Family Advisory Groups. Each group will be regionally located according to HealthChoices regions and will meet on a quarterly basis. The Pediatric Coach will be responsible for coordinating all aspects of these events

Qualifications:
  • Bachelors degree in one of the following: social work, learning and instruction, human services or related field
  • Life experience preferred in working with children with complex medical needs and families
  • Life experience preferred in working with Managed Care Organization (MCO), Supports Coordination, Administrative Entities and/or medical service providers
  • Knowledge of public health care as it relates to people with Intellectual Disabilities, Developmental Disabilities and Autism.
  • Intellectual Disabilities systems knowledge preferred
  • Act 33 and Act 34 clearance and FBI background check, if applicable
  • Valid PA Driver's License
Minimum Compensation:USD $45,000.00/Yr.Maximum Compensation:USD $60,000.00/Yr.
