The Cottages of Dartmouth Village has immediate opening for a Health and Wellness coordiantor with assisted living and memoray care experience who is dedicated to the care and wellbeing of our residents. Position Summary:

Manages the day-to-day healthcare operations of the community to ensure residents healthcare needs are met. Ensures residents are treated with respect and dignity and ensures quality care as residents healthcare needs change.

Esstential Duties Include:

• Assists in the supervision of community-based licensed nursing staff and Resident Care Associates
• Assists in the maintenance of resident charts and review of documentation performed by care giving staff
• Ensures and conducts Personal Service Plan reviews and updates, according to Charter Senior Living and state requirements, to ensure residents' services are met
• Communicates information in a timely manner to the appropriate person or agency regarding changes in resident's health/well-being, personal needs, risk management issues, customer service issues, family or outside health care provider concerns.
• Shares on-call and Manager or Duty responsibilites / shifts as required

Education and Experience

Must posses a valid Massachusetts LPN or RN license

Minimum of 2 years relevant experience.
