Job Description:

Equi-Venture Farms, LLC is rapidly growing and were seeking an experienced Health Care Coordinator to provide exceptional healthcare services to our participants. Our ideal candidate will have excellent organization and communication skills, and 1-3 years of experience working as an HCC or similar classification. Youll assist our participants with health care coordination, companionship, basic medical care, personal care, and medication delivery/audit so that they are empowered to live happily within the community. Responsibilities:

Mitigate issues with medications, organize self-administered dosing, arrange for refills, and monitor resident nutrition, meal preparation, and planning.

Maintain psychological and emotional support for participants and family members - providing quality companionship.

Transport participants as needed for doctor's offices or other health care services, or for other other errands.

Implement ambulation methods, and assist participants in completing physical therapy and exercises as indicated in the care plan.

Reinforce accurate and up-to-date care records and work with the team to implement the resident care plan.

Complete basic medical tasks such as taking vital signs, measuring glucose and ketone levels, replacing bandages and dressings, and ensuring the proper use of medical devices.

Provide personal care tasks such as calling doctor's offices, scheduling appointments, coordinating Medicare and other insurance details.

Qualifications:

Must have at least 1 year of experience as a Personal Caregiver, Home Health Aide, Nursing Assistant, or Personal Care Aide for a home health agency at patient homes or care facilities setting.

Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) is a Bonus.

Pass a full employment background check.

A high school diploma or GED equivalent is required - some college is an asset.

Carry a valid State Drivers License and have reliable transportation.

Compassionate, kind, and empathetic personality is essential, with superb organization and communication skills.

Proficient in Word, Excel, Outlook, data entry, etc.
