Learn more at www.voala.org.PAY RATE: $19.95 - $21.00 Per HourBENEFITS: VOALA offers competitive medical, dental, vision and retirement benefits.JOB SUMMARY AND PURPOSESSVF Health Care Navigators connect Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. SSVF health care navigators provide case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties. SSVF The VOALA health care navigator will act as a liaison between VOALA and the VA or community medical clinic and works with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans.DUTIES AND RESPONSIBILITIES* Non-Clinical Assessment* Assess clients for participation in various government benefit programs, the incumbent conducts assessments of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members, and significant others. The purpose of the assessment is to understand the Veterans situation, potential barriers to care, the causes, and the impact of such barriers on the Veterans ability to access and maintain health care services. Works with participants to develop Individual Needs and Services Plans (INSP) to navigate those governmental benefits programs.* Health Care Team and Veteran Communication* Works closely with Veterans to assist them in communicating their preferences in care and personal health-related goals to facilitate shared decision making of the Veterans care.* Serves as a resource for education and support for Veterans and families and helps identify appropriate and credible resources and support tailored to the needs and desires of the Veteran.* Specialized Case Management and Care Coordination* The incumbent provides comprehensive case management and care coordination across episodes of care* Coordinates case conferences with residents, consults with service team, monitors progress of clients on Individual Needs and Services Plans (INSP), assists clients to meet the agreed upon goals.* Acts as advocate for client before judicial, community, social service, and administrative bodies as needed; facilitates support groups and client participation as appropriate.* The SSVF Health Care Navigator assists clients to access community resources related to the needs of the Veteran. The health care navigator collaborates with other providers in the ongoing reassessment of the Veterans health care needs. The health care navigator educates the Veteran and caregiver of the available services and assisting them in establishing the appropriate referrals based on the Veterans preference.* Health Education* Assists in identifying the Veteran and family's health education needs and provides education services and materials that match the health literacy level of the Veteran.* Interdisciplinary Collaboration, Coordination and Consultation* To ensure the best possible care, the incumbent collaborates with other disciplines involved in providing care. The incumbent regularly consults with other team members and appropriately assesses and addresses the needs of the Veteran. The incumbent understands the different roles within the interdisciplinary team and acts within professional boundaries.* Administrative Duties and Systems Improvement* Develops positive workplace relationships with staff and peers by participation in professional development events and offering appropriate support while maintaining a high level of professionalism* Participates in routine clinical supervision or consultation with Clinical Director* Attend staff meetings, case conferences, required training to ensure continuum and quality of care.* Assist with training and education projects for staff, residents and community* Serve as role model through positive, appropriate conduct and participation in agency events* Other duties as may be requiredQualificationsREQUIREMENTS:* Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verificationsEDUCATION:* Associates Degree in Social Services or related discipline, Or* Equivalent combination of education and work experience (Two years)EXPERIENCE:* Two years experience working with veterans, people in recovery and / or homelessness individuals, of which one year includes dealing with Veterans Administration or other Federal assistance programsPREFERRED QUALIFICATIONS:* Bachelors Degree (Social Work, Public Health Administration, or related)* U.S. Veteran strongly preferred* Existing relationships with VA or other community / veteran resource providers* HMIS Database experience strongly preferredVolunteers of America is an Equal Opportunity/Affirmative Action employer.
