Experience with the aged and aging population, especially in the provision of direct patient care as a human services or clinical professional, and an ability to appreciate how the aging process affects outcomes, and flexibility and dedication. Clinical knowledge and experience with Long Term Care and the aging population. Good understanding and use of Microsoft Office Suite, particularly Word, Excel, and Outlook, as well as SharePoint. Excellent written and oral communication skills. Federal Surveyor Minimum Qualifications Test certification and investigatory experience.Duties Description This incumbent will function as a complaint investigator, conducting interviews of residents, family representatives and the Interdisciplinary team; reviewing medical records, care plans, discharge planning; and performing quality assurance. Additionally, the incumbent will conduct post survey revisits both onsite and offsite and be involved in immediate jeopardy and healthcare emergency situations as they arise. The incumbent will write and review statements of deficiencies and case summary reports, ensuring consistency with applicable federal requirements, general grammar principles and evidence obtained; ensure all reports are appropriately entered into the applicable database, consistent with the applicable terms of use; and will be responsible to review and recommend approval of facility-prepared plans of correction and where applicable, conduct post-survey revisits to verify compliance.Additional Comments This position requires travel at times to areas unserved by public transit. The successful incumbent must be flexible and available to work varied hours, which may include weekends and holidays, to respond to emergencies and/or provide guidance relative to off-hours investigations, as needed. Federal Surveyor Minimum Qualifications Test certification is required within one year of date of employment.Posted salary range includes the statutory hiring rate at Step 4 and job rate for PS&T Grade 20 ($81,720-$91,381) plus an additional Geographic Pay Differential of $35,000 and an additional $3,400 location pay.This position is part of the NYS HELPS program. No examination is required.
