Position Summary:The care coordinator, in collaboration with the healthcare team, coordinates the plan of care for the patient and family through the continuum of care, both for patients seen in the health centers as well as hospital discharges. This role is one of coordination of services, data collection, analysis, and monitoring of resource utilization with a systematic approach to interventions for improved patient outcomes, clinically, financially, and with patient satisfaction. Care coordinator is trained in connectivity to health care coverage, community primary and preventative services, social support resources, and patient education related to these topics. They are experts on available programs and resources and are aware of and sensitive to the unique needs of the patients, families, and community.Educational Requirements/ Qualifications: Must have a current NYS RN license. Case management/care coordination experience preferred. Requires excellent communication and collaboration skills. Requires ability to problem solve in complex situations. Must be able to work around interruptions and cope with stressful situations.
