We focus on building relationships and collaborations with community partners to deliver primary healthcare and improve the overall personal health and well-being for all residents of Knox and surrounding counties. The Health Insurance Navigator will become a certified Insurance Navigator through the state of Indiana. They will work with all self-pay patients to obtain Medicaid, commercial health insurance coverage via Marketplace, or a sliding fee scale discount based on the federal poverty guidelines. The Health Insurance Navigator will coordinate with other community agencies to help a patient with any type of socioeconomical deficiencies. They will continue to work with the patient as long as the patient needs any type of assistance.This position will work from the Daviess County Family Health Center and will travel as needed to the Martin County Family Health Center.ESSENTIAL DUTIES AND RESPONSIBILITIESThe essential functions include, but are not limited to the following:1. Successfully complete online Navigator Training and Certification during the initial training period and maintain annually thereafter2. Continuously comply with all required Navigator duties as outlined by CMS3. Protect personally identifiable information (PII) to ensure that confidentiality is maintained in accordance with HIPAA regulations4. Assist patients with gaining a sliding fee scale discount by obtaining all required documentation to support the Federal Poverty Level guidelines for a household income5. Assist patients with Medicaid applications to enroll as presumptive coverage or full Medicaid as well as look at options for Marketplace coverage6. Work with FHC billing department and central registration when insurance has been obtained as presumptive, full Medicaid, or a Marketplace coverage for entry into the patient's registration in EPIC7. Monitor monthly all self-pay patients to see if Medicaid was obtained via a different source, so the patient's insurance can be billed8. Help schedule all patient's appointments, new and/or follow-up, with the appropriate provider based on the type of service and insurance9. Screen all behavioral health patient calls for suicidal symptoms prior to scheduling their appointment 10. Identify and establish collaboration with community-based organizations and community sites for enrollment assistance, outreach and education events11. Provide wrap around services when patients need assistance linking to other community agencies12. Monitor and follow-up with all after-hours voice messages from patients13. Ability to work well with all types of patients and staff, strong verbal, and written skills as well as motivational skillsc.
