Qualifications:The Health Educator is responsible for performing a variety of tasks including outreach and enrollment services for various programs for high-need families in underserved communities. The Health Educator is an important participant in the continuum of care and contributes to health promotion and disease prevention by connecting individuals and families to appropriate classes, health access, preventive care, oral health services, 0-5 programming, behavioral health support and other allied health services to improve health outcomes.The Health Educator is an integral member of the Health Education and Outreach Department and works closely with medical providers, clinic staff, patients, community members and volunteers. This person must be able to communicate effectively and professionally within the health care environment as well as with the residents in the community. The Health Educator must be willing to perform grassroots activities in order to achieve the larger goals and objectives of TCC Family Health.
Qualifications:- Bachelor's degree required
- Entry level experience
- Understanding of effective primary prevention
- Experience working with diverse and under-served populations
- Bilingual Spanish/English or Khmer/English preferred
- Excellent speaking, writing and analytical skills
- Ability to provide respectful, empathetic and excellent customer service
- Ability to coordinate multiple projects and deadlines in a high-quality manner with attention to detail
- Ability to work and thrive in a fast-paced, dynamic environment; multi-task and shift priorities as needed
- Desire to achieve significant prevention outcomes and commitment to reducing health disparities
- Ability to maintain a high level of professionalism and confidentiality
- Proficiency in Windows-based PC environment (word processing, spreadsheet, database, Internet).
- Ability to work afternoon hours, evenings and weekends
- A valid California Driver's License, proof of insurance and access to a car required.
