Overview:

The Health Home Adult Care Manager Supervisor plays a crucial leadership role within the Health Home program, overseeing the delivery of integrated care services to adult clients with complex medical, behavioral, and social needs. This position involves managing a team of team leads and care managers, guiding them in the development and execution of individualized care plans, and ensuring the highest quality of care through evidence-based practices. The Supervisor is responsible for maintaining program compliance with Department of Health (DOH) regulations, promoting a client-centered approach, and driving initiatives that enhance health outcomes and reduce barriers to care.

This role requires a proactive leader who can balance clinical expertise with strong management skills, fostering a supportive environment where care managers are empowered to reach their full potential. The Health Home Adult Care Manager Supervisor collaborates closely with healthcare providers, community organizations, and other stakeholders to build a seamless network of services for clients. Responsibilities:

  • Team Leadership and Supervision:

    • Lead a team of six Team Leads who supervise eighty adult Care Managers, offering mentorship, guidance, and performance evaluations.
    • Conduct regular one-on-one meetings and team reviews to provide support and address challenges.
    • Lead weekly/monthly team meetings to ensure open communication, share best practices, and encourage continuous learning.
    • Review caseloads and develop personalized strategies to achieve performance targets.
    • Collaborate with the director to create action plans for underperforming Care Managers and provide targeted coaching.
  • Program Oversight:

    • Monitor care management activities for compliance with state and organizational guidelines, including Health Home and Medicaid regulations.
    • Ensure all Care Managers maintain accurate, timely, and thorough documentation in the EHR system.
    • Review case files and service plans to ensure adherence to evidence-based practices and quality standards.
    • Implement workflows to improve service delivery and reduce inefficiencies.
  • Care Coordination and Quality Assurance:

    • Oversee the development and evaluation of person-centered care plans for adult clients.
    • Coordinate team meetings and case conferences to address complex cases and improve client outcomes.
    • Analyze care management data to identify trends, service gaps, and opportunities for improvement.
    • Support Care Managers in crisis intervention planning and adherence to safety protocols.
  • Training and Development:

    • Monitor training sessions for new Care Managers, ensuring they follow the schedule and gain access to the EHR system.
    • Provide ongoing professional development opportunities focused on motivational interviewing, trauma-informed care, and chronic disease management.
    • Stay informed on industry trends and healthcare regulations to ensure the team delivers top-notch services.
  • Client and Community Engagement:

    • Act as a liaison between Care Managers, clients, and external partners to promote integrated care coordination.
    • Handle escalated client issues professionally to improve client satisfaction.
    • Participate in community outreach activities to promote the Health Home program.
  • Data Management and Reporting:

    • Oversee quality metrics and reporting, ensuring accurate tracking of client outcomes and service utilization.
    • Collaborate with the director to monitor dashboards and Care Managers' performance.
    • Prepare reports on team performance, client progress, and areas for improvement.
  • Recruitment and Screening of New Care Managers:

    • Collaborate with Intake and Recruitment on the onboarding process for new hires, including conducting interviews and evaluating candidate fit.
    • Ensure thorough and consistent onboarding/offboarding with HR, ensuring proper access to resources and compliance.
    • Participate in new hire onboarding, providing necessary tools and resources for a smooth transition.

Qualifications:

  • Education:
    • Bachelor's degree in Social Work, Human Services, Public Health or a related field required
  • Experience:
    • Minimum of 3 years of experience in care management, care coordination, or case management in a healthcare or social services setting.
    • At least 1 year of supervisory or leadership experience preferred.
  • Skills and Abilities:
    • Strong understanding of health home and care management principles for adult populations.
    • Excellent leadership and team-building skills, with the ability to mentor and motivate staff.
    • Effective communication and interpersonal skills, both written and verbal.
    • Proficiency in care management software and Microsoft Office Suite.
    • Ability to work in a fast-paced environment while managing multiple priorities.
