Job Type Full-time Description Title: Intake Specialist, Health Homes Department: Care Management Reports To: Member Engagement Coordinator Location: 56 Bay Street, Staten Island, NY 10301 Position Status: Full Time FLSA Status: Non-Exempt Pay Rate/Benefits: The pay range for this role is generally $37,000 - $41,000 annually, commensurate with relevant experience and qualifications and in alignment with internal equity. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities - the poor and working poor, low income people with chronic illnesses, persons with criminal justice involvement, substance users, domestic violence survivors, people of color, and the LGBTQ community. POSITION SUMMARY: The Health Home program ensures persons living with chronic illness get the care and services they need. Individualized to the client, this may mean fewer trips to the emergency room or less time spent in the hospital, getting regular care and treatment from providers, or obtaining food and a safe place to live. The Intake Specialist is responsible for the admission and enrollment process of new referrals to the Health Home program, including confirmation of program eligibility. The Intake Specialist works closely with the Health Home team to identify potential referrals for the program and conduct outreach efforts. Outreach efforts include conducting phone calls, mailing letters, following up with referral sources, and attempting home visits to conduct enrollment. Intakes may require face-to-face interaction, as some may need to be completed in the provider's office or in a person's home. As necessary, the Intake Specialist participates in agency and community events where they would promote Health Home services and develop relationships with other service providers. DUTIES & RESPONSIBILITIES:

  • Receive and manage new referrals for Health Homes program.
  • Conduct intakes and ensure completion of intake documents and initial assessments.
  • Ensure Medicaid and program enrollment eligibility through use of EPACES, MAPP, and PSYCKES systems.
  • Admit new intakes in Foothold Care Management electronic health record and upload all supporting documents as per Lead Health Homes policies and procedures.
  • Support Care Management and team activities as needed.
  • Complete community-based work to conduct enrollments and promote program services.
  • Receive appropriate training to maintain clinical and programmatic standards.
  • Participate in department meetings and clinical reviews.
  • Conduct administrative tasks as needed.
  • Perform all functions in alignment with CHASI's Mission, Vision, and Core Values.
  • Perform other duties as requested.
Requirements QUALIFICATIONS:
  • At least 1 year of related experience required.
  • High school diploma or equivalency required.
  • Strong socio-cultural identification with the target population.
  • Good customer services skills with welcoming attitude and can-do spirit.
  • Bilingual (English/Spanish) preferred.
  • Detail orientated.
  • Knowledge and understanding of care management programs.
  • Excellent oral and written communication skills.
  • Excellent interpersonal skills.
  • Good problem-solving, decision-making and judgment skills.
