- Establish trusting relationships with clients to provide ongoing motivation and positive feedback regarding client changes or progress.
- Interact with clients on phone or in person on a regular basis.
- Meet with clients before and after appointments to assist client understanding and access to resources to comply with provider instructions.
- Identify barriers to care and client's goals.
- Conduct wellness groups as appropriate.
- Assist with care coordination to clients.
Need to have:
- Community Health Worker certificate.
- A valid driver's license, the ability to obtain a and daily access to your own vehicle sufficient to transport clients to appointments and showings and for travel to various shelters and drop-in centers throughout Hennepin County.
