**Job Overview**

**Description**

Medical Associates is seeking a full time **Health Information Assistant** ! This position contributes to the delivery of Release of Information to clinic providers, departments, and patients. Additionally, this position will provide support for medical records coming into the clinic.

**Skills You Bring:** Previous customer service experience, office experience including computer skills, answering phone calls, attention to detail. Ability to multi task and utilize multiple computer applications. Medical terminology and healthcare experience of any kind are helpful, but not required.

**Essential Functions:**

+ Receive/process request for release of information using established procedures, guidelines and regulations maintaining turnaround time. Validate authorizations. Release via third party or directly to requesting party. Scan and/or import personal health information (PHI) into electronic health record (EHR), assigning appropriate indices to the scanned image per general guidelines in accordance with time frames and deadlines. Appropriately access, analyze and perform quality checks on pertinent PHI to ensure accuracy.

+ Respond/correspond to requesting parties, clinical/non-clinical areas, and patients regarding various issues, by providing information and clarification as needed. Identify any discrepancies and recognize errors. Research and problem solve by effectively demonstrating in-depth knowledge of processes, guidelines, and numerous software applications. Seek resolution.

+ Identify invalid forms or requests to release information. May make decision to exceed general guidelines. Advise requesters if requests cannot be met, giving reasons. Accurately locate, retrieve, send and re-file paper medical records using tracking system/terminal digit system.

+ Complete all other assigned projects and duties. Implement features of retention/destruction of patient information per Clinic guidelines.

+ Communicate regularly with Lead, Manager, or Director. Update Lead and Manager on workloads and any potential backlogs.

**Knowledge and Skills:**

Experience: From three months to one year of similar or related experience.

Education: Equivalent to a high school diploma or GED.

Interpersonal Skills: Courtesy, tact, and diplomacy are essential elements of the job. Work involves personal contact with others inside and/or outside the organization, generally regarding routine matters for purposes of giving or obtaining information which may require some discussion.

Other Skills: Working knowledge of medical terminology and forms. Effective utilization of third party applications. Ability to organize, prioritize workload; make good judgments. Consistently demonstrate initiative. Exercise effective telephone skills.

**Physical Aspects:**

Reaching - Extending hand(s) and arm(s) in any direction.

Lifting - Raising objects from a lower to a higher position or moving objects horizontally from position-to-position. This factor is important if it occurs to a considerable degree and requires the substantial use of the upper extremities and back muscles.

Fingering - Picking, pinching, typing or otherwise working, primarily with fingers rather than with the whole hand or arm as in handling.

Grasping - Applying pressure to an object with the fingers and palm.

Talking - Expressing or exchanging ideas by means of the spoken word. Those activities in which they must convey detailed or important spoken instructions to other workers accurately, loudly or quickly.

Hearing - Perceiving the nature of sound with or without correction. Ability to receive detailed information through oral communication and to make fine discriminations in sound, such as when making fine adjustments on machined parts.

Vision - 20 / 40 or better in the best eye with or without correction.

Repetitive Motions - Substantial movements (motions) of the wrists, hands and/or fingers.

Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for Sedentary Work and the worker sits most of the time, the job is rated for Light Work.

Environmental Conditions:

None - The worker is not substantially exposed to adverse environmental conditions (such as in typical office or administrative work).
