Within DCF, the Substance Abuse and Mental Health (SAMH) Program office is recognized as the single state authority for substance abuse and mental health services. The Office of SAMH is statutorily responsible for the planning, evaluation, and implementation of a comprehensive statewide system of care for the prevention, treatment, and recovery of children and adults with serious mental illnesses or substance use disorders.
Northeast Florida State Hospital (NEFSH) is a 613-bed, licensed mental health treatment facility that provides person-centered services to adults suffering from severe and persistent mental illness. The hospital, located in Macclenny, was established in 1959 and is the largest state operated civil treatment facility.

This is a highly responsible and professional position that will serve as a Health Information Specialist at the Northeast Florida State Hospital. T his position will collaborate with each section within the state facilities and be required to use independent judgement, take initiative in carrying out duties and responsibilities and should be extraordinarily detail oriented.
Specific Duties and Responsibilities include:
+ This is a responsible position that requires independent judgement and decision making assigned to the Health Information Services Department of a large mental health treatment facility. To ensure treatment and management excellence the incumbent in this position must understand the importance of the informed consent and NEFSH operating procedures relevant to the assigned duties and responsibilities.
+ Assists the psychiatrists with obtaining informed consents, emergency treatment orders (ETO), and court authorization for the use of psychotropic medication in accordance with NEFSH OP 14-06-01 and CFOP 155-1.
+ Types all correspondence associated with the above processes, acts as a witness for the provider receiving verbal consent and handles the sending out of consents for signature via certified mail.
+ This task includes ensuring that all informed consents are thoroughly reviewed to determine completeness, accuracy and conformance with hospital requirements and state and federal regulations that relate to both the provision of Medical Records services and the documentation practices of the staff.
+ Follows up with providers when discrepancies are found and ensures necessary corrections are made.
+ Reviews patient medication orders to ensure that the current consent matches the medications being ordered.
+ This is a task that requires a high degree of scrutiny and attention to detail.
+ As new medications are ordered every day this is a task that has to be completed consistently to stay up to date.
+ Follows up with providers when discrepancies are found and ensures necessary corrections are made.
+ Tracks and monitors the receipt of completed informed consents and court orders.
+ Maintains an electronic log for each living area to ensure the consents are completed and updated per time requirements.
+ Shares this information with the psychiatric providers to keep them up to date on what is due.
+ Maintains the Informed Consent drive to ensure accuracy, organization and availability for multiple departments.
+ Sorts paperwork related to the informed consents and and provides a hand off to the Health Information Services department to ensure the documents are filed in the patient's chart.
+ Provides data to the Quality Improvement Department and the Medical Services Department on the completeness and accuracy of informed consents.
Knowledge, Skills and Abilities required for the position:
+ Knowledge of the procedures for securing, accumulating, processing, preserving, retrieving and controlling the distribution of medical records.
+ Knowledge of the procedures for coding, classifying, indexing and filing medical records manually and/or electronically.
+ Ability to use a nomenclature and classification system.
+ Ability to plan, organize and coordinate work assignments.
+ Ability to communicate effectively.
+ Ability to establish and maintain effective working relationships with others.
Minimum Qualifications:
+ Certification as an Registered Health Information Technician (RHIT) by the American Health Information Management Association (AHIMA); or
+ Three years of experience in compiling, maintaining and coding medical records.
