- High School diploma or equivalent.
- Psychiatric/Behavioral Health experience preferred.
- Healthcare technology experience preferred.
- Ability to perform basic medical assistant job duties.
Core Job Duties
- Manage and oversee a caseload of clients, focusing on primary care screening and monitoring.
- Ensure clients undergo all initial and ongoing screenings as outlined in the Primary Care Screening and Monitoring Policy.
- If necessary, conduct health screenings to identify potential risks and address them proactively.
- Obtain screenings conducted by the client's Primary Care Physician (PCP), if available, and scan them into the Electronic Medical Record (EMR) system.
- Provide ongoing health education to clients based on identified conditions.
- Make and track referrals to primary care and specialty services.
- Assist clients in scheduling primary care and specialty services appointments when necessary.
- Monitor attendance at external appointments to assess whether clients followed through with scheduled visits.
- With the client's consent, coordinate care with primary care and specialty health providers.
- Encourage clients to adhere to their PCP's treatment plan and recommendations.
- Collaborate with the client's FCC providers to develop comprehensive care plans/approaches for mutual clients.
- Document all education provided, referrals made, assistance with appointment scheduling, and collaboration with the PCP or other providers.
- Adherence to all applicable evidence-based practice models.
- Orally communicate information effectively and accurately.
- Assure program operates within the constraints of the agency's certification (CARF, DMH) standards and strategic plan related to clinical care.
- Abide by program and agency policies and procedures.
