Location: Georgetown, DE - onsite
Job Summary:
Assist patients in qualifying for state resources and coordinate healthcare program applications, primarily in Georgetown with occasional travel.
Essential Responsibilities:
- Help uninsured/underinsured patients enroll in medical assistance programs.
- Screen and counsel patients on financial eligibility.
- Promote health screenings and educate patients on healthcare participation.
- Develop outreach strategies and inform physicians about available services.
- Assist with applications, schedule screenings, and follow up.
- Maintain records, ensure HIPAA compliance, and report outreach data.
Education and Experience:
- Associate's degree or equivalent experience.
- At least one year of healthcare experience.
- Experience with community resources, case management, and record-keeping.
